Case study: Patient-Centered Care: Case Studies on End of Life in elderly
Background
Ms. L is an 87-year-old African American woman who was diagnosed with vulvar cancer at the beginning of 2017. She is also HIV-positive. By the time Ms. L engaged in care, the cancer had proliferated quite quickly in the setting of a compromised immune system. Upon discovery of the Stage 4 cancer, doctors recommended a dose of radiation and chemotherapy. However, during the course of this episode Ms. L was struggling with substance use. During her hospitalization, she tested positive for a number of substances, including heroin and cocaine. As a result, care providers had many discussions about pain management and which pain medications could be given to her. She was not on methadone treatment maintenance at first, so she was self-medicating to address her pain. While Ms. L wanted to seek help for her addiction to substances, some of the traditional models were not appropriate given the magnitude of her physical issues. There were expectations that she would get into outpatient treatment but she did not follow through, primarily because it was difficult for her to tolerate being in groups for long periods. (Given the location of her cancer, she could not sit upright for long periods or on the bus for transportation.) Ultimately, Lawanda Williams, Director of Housing Services at Health Care for the Homeless in Baltimore, Maryland,.

also HIV-positive. By the time Ms. L engaged in care, the
cancer had proliferated quite quickly in the setting of a

about pain management and which pain medications could be

at first, so she was self-medicating to address her pain. While
Ms. L wanted to seek help for her addiction to substances, some
of the traditional models were not appropriate given the
magnitude of her physical issues. There were expectations that
she would get into outpatient treatment but she did not follow
through, primarily because it was difficult for her to tolerate
being in groups for long periods. (Given the location of her
cancer, she could not sit upright for long periods or on the bus
for transportation.) Ultimately, Lawanda Williams, Director of
Housing Services at Health Care for the Homeless in Baltimore,
Maryland, and her team were able to provide Ms. L with
transportation and cab vouchers so she could access the full
course of radiation that doctors had recommended. Her pain was
never well controlled, because her physician refused to
prescribe her any pain medications, due to the magnitude of her
3. substance use. The radiation center gave her Percocet while she
was there but would not give her anything that could not be
directly supervised. After treatment, they sent her home with
prescriptions for Tylenol and instructions to return and follow
up with pain management teams, which she was unable to do
because of her difficulties with transportation and sitting. Ms. L
completed radiation and is in a period of holding to assess
effectiveness of the initial course of radiation, but she still does
not have a prescription for her significant pain and, as a result,
continues to use substances to manage her pain. Ms. Williams
observes, “I have been able to see how managing withdrawal
and managing substance abuse in the context of a palliative care
treatment plan does not always exist for patients experiencing
homelessness. She does not fit very neatly into any mainstream
treatment model.”
CASE STUDY CHALLENGE:
1. Harm reduction: How can care providers best advocate for a
harm reduction approach while seeking to deliver palliative care
services, including hospice care?
2. What ethical arguments can you make base on the case study?
3. Why do you think that long-term care and palliative care
insurance lacks of popularity among older Americans.
3.
Conclusion (15%)
Briefly summarize your thoughts & conclusion to your critique
of the case study and provide a possible outcome for Aging in
America base on Health ethics Context?
Evaluation will be based on how clearly you respond to the
above, in particular:
a) The clarity with which you critique the case study;
b) The depth, scope, and organization of your paper; and,
4. c) Your conclusions, including a description of the impact of
these Case study on any Health Care Setting.

Module 2 | Part 2: Strategies to Promote Academic Integrity and
Professional Ethics
I have analyzed the relationship between academic integrity and
writing, as well as the relationship between professional
practices and scholarly ethics. I have also identified strategies I
intend to pursue to maintain integrity and ethics of my academic
work while a student of the MSN program, as well as my
10. professional work as a nurse throughout my career. The results
of these efforts are shared below.
Directions: In the space below craft your analysis/writing
sample, including Part 1 (The Connection Between Academic
and Professional Integrity) and Part 2 (Strategies for
Maintaining Integrity of Work).
Part 1: Writing Sample: The Connection Between Academic and
Professional Integrity
In the space below write a 2- 3-paragraph analysis that includes
the following:
· Explanation for the relationship between academic integrity
and writing
· Explanation for the relationship between professional
practices and scholarly ethics
· Cite at least 2 resources that support your arguments, being
sure to use proper APA formatting.
· Use Grammarly and SafeAssign to improve the product.
· Explain how Grammarly, Safe Assign, and paraphrasing
contributes to academic integrity
Section 1: Academic Integrity in relation to Professional
Integrity
There are close relations involving academic integrity and
writing. The academic integrity refers to the
commitment towards upholding the highest standards of
intellectual honesty in all aspects of one's academic work
(Adhiambo, 2019). Academic integrity would include avoiding
plagiarism, cheating, and fabrication. On the other hand,
writing refers to the process of articulating an
individuals’ ideas through a written form. Academic integrity is
essential because it is the cornerstone of any educational
11. enterprise as it allows scholars to trust in their colleagues' work
and build upon it. Secondly, academic integrity is essential for
ensuring that students receive a fair and accurate assessment of
their work. Cheating and plagiarism undermine the validity of
grades and reviews and make it difficult for instructors to
evaluate students fairly. Academic integrity protects the
reputation of the individual and the institution (Adhiambo,
2019). There are several ways to maintain academic integrity in
one's writing: being honest about one's sources, ideas, and data,
following institutional guidelines, and avoiding plagiarism.
The connection between scholarly ethics and professional
conduct is complicated. Professionals or specialists are
generally mandated to adhere to ethics rules of conduct which
commonly regulate effective workforce. On the other hand,
scholarly ethics are concerned with ethical issues while
conducting research and writing papers. Maintenance of ethics
forms among the significant aspects of the nurses (Dehghani,
2020). In general, professional practices are more concerned
with the day-to-day conduct of professionals, while scholarly
ethics are more concerned with ethical research issues.
Professional codes of conduct typically set the
standards nursing professionals must uphold daily. Scholarly
ethics focus more on moral issues, including questions about the
appropriate use of data, the correct way to cite sources, and the
need to avoid plagiarism.
Grammarly, Safe Assign, and Paraphrasing contribute to
academic integrity by helping users produce original, well-
written work that is free of plagiarism. Grammarly is a software
application that checks for grammar and spelling
errors (Curtis& Tremayne, 2021). Grammarly can also be used
to check for plagiarism in both written and electronic sources,
making it an invaluable tool for preventing academic
dishonesty. Safe Assigns a similar software application that
checks for plagiarism in written and electronic
sources and online sources, such as websites and
journals. Paraphrasing is the process of rewriting a piece of text
12. in your own words. Paraphrasing may be a form of cheating and
can lead to severe consequences; therefore, it is essential to cite
all sources used in a paper or project. Cheating, plagiarism, and
other forms of academic dishonesty can lead to
serious ethical consequences.
PART 2: Strategies for Maintaining Integrity of Work
Expand on your thoughts from Part 1 by:
Identifying and describing strategies you intend to pursue to
maintain integrity and ethics of your 1) academic work while a
student of the MSN program, and 2) professional work as a
nurse throughout your career. Include a review of resources and
approaches you propose to use as a student and a professional.
A Master of Science Student in nursing can use different
strategies to maintain the integrity and ethics of their academic
work. Such strategies include adhering to the codes of conduct
and ethical principles of the nursing as a career (Bah & Sey-
Sawo, 2018) as well as being honest and transparent in all
aspects of their academic work. Other strategies are refusing to
engage in any form of academic dishonesty or cheating and
ensuring that all sources used in academic work are properly
cited. Safe Assign is one of the resources that, as a student,
I would use to keep checking my academic work to
avoidplagiarism that would bring integrity issues.
Additionally, there are many strategies that a nurse can use to
maintain the integrity and ethics of their professional work.
Some strategies include adhering to
the ethics requirements set out by American Nurses Association
(ANA) that provides nurses with a set of principles and values
to guide their professional practice. Secondly, keeping up with
13. the latest research and developments in the nursing field and
remaining honest and transparent in professional dealings (Bah
& Sey-Sawo, 2018). Abiding by the ANA provisions is one
approach that facilitates professional practice as a nurse.

Notes
Reliable
Poor generalizability as the research subjects were recruited
from villages in Tianjin, China.
The article relates to my topic of interest.
The research tries to discover the differences between men and
women in the prevalence of diabetes and its causes in a poorly
educated population in China.
Contain consistent data
Research participants were only evaluated for FPG (fasting
plasma glucose) levels; without glycosylated hemoglobin data
and impaired glucose-tolerance testing, the findings may have
underestimated the risk of diabetes.
The information contained in the article is useful to my topic of
interest
The findings suggest the need to implement customized weight,
blood pressure, and lipid management among lower-income
groups in China to reduce the risk of diabetes among older
women.
Easy to analyze
16. The reliance of the research on self-reported diabetes
underestimated the prevalence of the disease.
The article is scholarly – it has gone through a peer review
process.
The prevalence of diabetes was higher among women (14.5
percent) and lower among men (14.1 percent).
Relatively easy to understand
The self-reported salt intake may affect the accurate analysis of
the relationship between salt intake and risk for diabetes since
the data was collected from a poorly-educated population.
The article is suitable for a research paper.
The research study further discovered that obesity, overweight,
high triglycerides, and hypertension were the major risk factors
for diabetes in both men and women.
Accurate
Data on nutrition and socio-economic status were not collected
due to poor education among the target audience.
The research article provides more insights into the prevalence
of diabetes among men and women.
In women, physical activity, advanced age, and high LDC-C
levels were additional risk factors for diabetes.
Valid
The article does not omit important information.
In men, the independent risk factor was a high HDL-C level.
Clear
The authors are credible – they are experts in the field of
17. endocrinology and neurology.
Every author read and ratified the final, peer-reviewed
manuscript.
Verifiable
The research article was intended to inform healthcare
professionals.
Researchers obtained written informed consent from
participants.
Sequential
The information presented does not contradict what I know.
The medical ethics committee at Tianjin Medical University
approved the study.
Concise
The methodology used for the research is described.
There were no any financial or commercial relationships that
could be interpreted as a potential conflict of interest in the
research.
I would recommend this article to inform professional practice.
Although it has some limitations, it is reliable, relevant, easy to
understand, and easy to analyze. It also contains consistent data
and can be used in the diagnosis, treatment, and management of
diabetes – one of the deadliest diseases in the globe.
Step 2: Summary of Analysis
Craft a summary (2-3 paragraph) below that includes the
following:
·
18. Describe your approach to identifying and analyzing
peer-reviewed research
·
Identify at least two strategies that you would use that
you found to be effective in finding peer-reviewed research
·
Identify at least one resource you intend to use in the
future to find peer-reviewed research
The word “scholarly” simply means that a journal article has
been peer-reviewed. This is a long review and editing process
carried out by scholars and experts in a particular field to
determine validity and quality of a journal (Northcentral
University Library, 2022). My approach to identifying and
analyzing peer-reviewed research is quite simple. I usually go
to the journal’s website and find information on their revision
and submission guidelines. Sometimes I search the title of the
journal in UlrichsWeb Global Serials Directory to get more
information about the journal.
One major strategy I would use in finding peer-reviewed
research is using a library database. Most of the databases are
limited to peer-reviewed research or allow users to limit their
search to peer-reviewed research. The other strategy is using a
Web Search engine, such as Google Scholar, to find peer-
reviewed research. In the future, I will use Scopus and JSTOR
to find peer-reviewed resources.
