This document is a service agreement between Superior Events and a client for go-go dancers at an event. It details the client's contact information, event date and time, hourly rates for two go-go dancers at $150 each with a 4 hour minimum, taxes and gratuity amounts, cancellation policy requiring payment if canceled same day, and terms requiring the client to pay invoices and assume risks of their equipment use. The client must sign and return with a 50% deposit to secure the event date.

Cancellation Policy: 2.5 Hours of “show” time will be billed to our client for each
employee canceled on the same day of the scheduled shift. 2.5-hours “show-up” time will
be billed for each employee canceled.
The Event Service Confirmation is "A Service Agreement" between "Client"
and Superior Events. Client agrees to pay (Superior Events) the agreed rate
for these hours upon receipt of invoice. Therefore, in consideration of this
service being made available to Client, Client agrees; 1. ( Superior Events) is
not responsible for Clients equipment failure or damage by (Superior Events)
employees working under Clients supervision. Further, (Superior Events)
employees are not authorized to handle cash or operate motor vehicles; any
such use by Client is done so at Clients full risk. If account is forced into
collection, all legal and court fees will be paid by client.
NOTE: PLEASE REVIEW THIS EVENT SERVICE CONFIRMATION SHEET TO CONFIRM
THAT ALL INFORMATION IS CORRECT. FURTHER, WE WILL NOT BE ABLE TO SECURE
THE DATE OF EVENT UNTIL WE HAVE RECEIVED YOUR SIGNED CONFIRMATION AND
50% DEPOSIT.
