1. Hr performance evaluation
Before you perform your first employee performance review you need to have the proper
paperwork. This "official" document should have some basic information to help in the
review process.
The first thing you will want on your performance review is the company letterhead.
Underneath the letterhead you should have in smaller letters the words "Employee
Performance Review".
The next section should be the employee information. You will want their name, ID
number if applicable, job title, what department they are in, date of the review, mangers
name, and review period date.
Then you will want a section for ratings. I would suggest the following as basic
categories; job knowledge, work quality, attendance/punctuality, initiative,
communication/listening skills, and dependability. Each category should be rated
between 1 for poor and 5 for excellent. You should also have a comment section for each
category. There should also be an overall rating which would average out all the separate
category ratings.
You should also have a separate area for general comments that may not have anything to
do with the rating categories.
It is also good to have an area for "goals". This section should be used to list out 2-3
separate goals that the employee should accomplish within the next 12 months. You
should also mention on the sheet that they can use the back side if they have too much for
the sections as printed.
Finally you should have a verification of review section. This is where both the employee
and manager will sign and date to make it official. It is also recommend having a
disclaimer to this effect; "by signing this form, you confirm that you have discussed this
review in detail with your supervisor. Signing this form does not necessarily indicate that
you agree with this evaluation."
It is very helpful if you print off two copies for each employee, one for them to do a self
evaluation and one for you the manager. The employee would turn theirs in as the
meeting gets under way. You may want to take a few minutes to look it over and then
discuss the differences after your official review is complete.
