This document contains a schedule change request form for Foley High School's Class of 2011. It provides instructions for students requesting a schedule change for the upcoming spring term, including requirements and procedures. Students must circle one or more reasons for the requested change on the form and obtain a parent's signature by December 14th. Approved changes will be reflected in the student's second term schedule distributed in homeroom after winter break. The form collects information about requested course drops and adds.

APPROPRIATE CHANGES WILL BE ATTEMPTED, BUT NOT GUARANTEED.
Electives are very limited and schedule changes WILL not be made simply because students request a “favorite”
teacher.

SCHEDULE CHANGE REQUEST REQUIREMENTS
A student may request a schedule change if he or she can answer, “YES” to one or more of the following
questions: YOU MUST CIRCLE ONE OR MORE OF THE FOLLOWING QUESTIONS to INDICATE
a “yes” response
1. Do you wish to increase academic rigor?
2. Are you scheduled for a course without having already passed a prerequisite course (example: you are
scheduled for Spanish II and you have not earned credit for Spanish I)?
3. Are you scheduled into a class that you have already taken and passed?
4. Are you unable to participate in a course because of physical limitations?
5. Is the course you are wishing to add a course REQUIRED for graduation?
SCHEDLE CHANGE PROCEDURES
1. The student should FULLY complete the Schedule Change Request Form
2. The student should list any additional reason(s) for the requested schedule change (on back of this form
below parent signature)
3. The student should obtain a parent/guardian signature
4. The student should LEAVE the form in Mrs. Neth’s box (by office door) by Monday December 14,
2009 at 3:30 PM.
5. If the schedule request is approved, then student will find the new changes made in their 2nd term
schedule which can be viewed online through STI or on the paper copy distributed in Homeroom on the
1st day of class 2nd semester.

2. Please note: Both the Alabama High School Diploma and Alabama High School Diploma with Advanced
Academic Endorsement REQUIRE 24 credits. The major difference in the two diplomas is that the Alabama
High School Diploma with Advanced Academic Endorsement REQUIRES one credit in Algebra II with
Trigonometry and two Foreign Language Credits. Additional information regarding the Baldwin County
diploma options can be found online at bcbe.org If a schedule change will modify a student’s diploma track a
statement indicating the new diploma choice SIGNED by a parent/guardian will be required before schedule
changes can be made.
