AADDVVIISSOORRYY::
At approx. 10:30 a.m. April 14, a light vehicle 01-3042 (Toyota Pickup) assigned to
the UG Drill & Support Equipment – Mine Maintenance Department hit a concrete
embankment at MP73 on the main road.
The vehicle was traveling from Amole Shop Besar – DOZ Portal to Underground
2510 Maintenance Shop with one passenger. The vehicle was near MP73 on the
downhill 25 km/hr (low speed zone) when the driver missed a gear while trying to
shift to 3rd
resulting in the vehicle running out of control and running into the
concrete embankment on the right side of the road.
No injury in this accident, both driver and passenger were using seat belts -major
damage to the engine and main frame.

1. Reported the occurrence to Safety Department and Command Center
2. Arrange the driver to the nearest clinic for alcohol test
3. Collected evidence and securing scene.
4. Revoke the driver license
RREEQQUUIIRREEDD AACCTTIIOONNSS::
Arrange general meeting with all team in UG Drill & Support equipment to
emphasizing the following:
1. Perform Pre-Operational Check, do not use and immediately report to
Supervisor if there any un-safe found during inspection.
2. Supervisor to ensure all Light Vehicle shall be refueled in at proper fuel/check
stations
3. Set up Investigation team to find out the root cause(s) of this accident for
corrective and preventive actions.
