Simulation for
Intramuscular
                     • The Simulation takes place in a
   Injection
                       simple Nursing Skills Lab
                     • There is a SIM Bot that
                       supports IV insertions and
                       blood draws as well as Proper
                       Injections (He is able to
                       communicate and give data
                       programmed )
                     • The Lab is set up in 3 sections
                     • The utility and supply section
                     • The patient room simulation
                       that contains a bed and the SIM
                       man and the computer
                       programmer)

Collection of supplies &        • The student is required to collect the
                                  appropriate equipment from the
Identification of patient         supply room (He is required to scan
                                  each product – the correct supplies
                                  will show a green light)
                                • The student is required to knock prior
                                  to entering the room (SIM man says
                                  come in- no knock SIM man says
                                  “excuse me”)
                                • The student introduces himself
                                • The student identifies the patient with
                                  two patient identifiers (SIM man
                                  Verifies his name and date of birth-
                                  student has patient MAR in hand and
                                  checks it against this data)
                                • The student explains the procedure
                                  and obtains consent from the patient.
                                • The student washes his hands and
                                  prepares the appropriate equipment

Proper procedure
      & Correct Site         • The student checks the MAR
Identification & Cleansing     against the prefilled syringe
                             • The student dons gloves
                             • The student identifies the
                               deltoid landmark (The SIM
                               man will respond-This is the
                               correct site for the Deltoid IM)
                             • The student then takes the
                               ETOH pad and uses
                               concentric circles to clean the
                               site (there is an applied
                               invisible film that will show
                               up when illuminated by black
                               light-proper cleansing will
                               remove the film)

The Correct       • The student grasps the
Injection
                    deltoid to firm up the
                    muscle
                  • The student removes the
                    cover from the syringe
                  • The student holds the
                    syringe with the thumb
                    and forefinger of the
                    dominant hand
                  • The student darts the
                    syringe in quickly at a
                    proper 90 degree angle
                    (the SIM man will say
                    “ouch” if the degree is
                    incorrect)

The correct       • The student removes
Injection           his non dominant hand
                    from the muscle, and
                    aspirates the plunger (if
                    incorrect placement-
                    syringe med color will
                    change and become
                    blue if in a vein, and
                    red if in an artery-if it is
                    in the muscle properly,
                    the syringe fluid color
                    will remain the same)

Simulation         • If no color change-the
Injection IM         student will push
                     plunger while
                     stabilizing the barrel
                   • The colored fluid will
                     be seen through the
                     plastic opening of the
                     deltoid muscle
                     demonstrating the
                     filling of the correct
                     muscle

Post procedure       • The student will
                       remove the needle
                       quickly & dispose of
                       needle in sharps box
                     • The student will
                       massage the area of
                       injection
                     • The student will assess
                       site for any bleeding
                       (Band-aid if small drop)
                     • .

• The student will assess
Post Procedue         tolerance of procedure from
                      patient (SIM man will provide
                      positive feedback if ALL steps
                      were followed correctly and in
                      proper sequence-if not-it will
                      identify areas requiring
                      improvement)
                    • The student will clean up and
                      through trash away
                    • The student will remove
                      gloves
                    • The student will wash his
                      hands
                    • The student will document
                      administration in the MAR

Assessment &         • The student will inquire
Documentation          if SIM man needs
                       anything else (response
                       will be no if all steps are
                       followed)
                     • The student will return
                       within the hour to check
                       patients response to
                       medication
                     • The student will
                       document efficacy of
                       intervention in the MAR

Collection of supplies & • The student is required to collect the appropriate equipment from the Identification of patient supply room (He is required to scan each product – the correct supplies will show a green light) • The student is required to knock prior to entering the room (SIM man says come in- no knock SIM man says “excuse me”) • The student introduces himself • The student identifies the patient with two patient identifiers (SIM man Verifies his name and date of birth- student has patient MAR in hand and checks it against this data) • The student explains the procedure and obtains consent from the patient. • The student washes his hands and prepares the appropriate equipment 3

Proper procedure & Correct Site • The student checks the MAR Identification & Cleansing against the prefilled syringe • The student dons gloves • The student identifies the deltoid landmark (The SIM man will respond-This is the correct site for the Deltoid IM) • The student then takes the ETOH pad and uses concentric circles to clean the site (there is an applied invisible film that will show up when illuminated by black light-proper cleansing will remove the film) 4

The Correct • The student grasps the Injection deltoid to firm up the muscle • The student removes the cover from the syringe • The student holds the syringe with the thumb and forefinger of the dominant hand • The student darts the syringe in quickly at a proper 90 degree angle (the SIM man will say “ouch” if the degree is incorrect) 5

The correct • The student removes Injection his non dominant hand from the muscle, and aspirates the plunger (if incorrect placement- syringe med color will change and become blue if in a vein, and red if in an artery-if it is in the muscle properly, the syringe fluid color will remain the same) 6
  • 7. Simulation • If no color change-the Injection IM student will push plunger while stabilizing the barrel • The colored fluid will be seen through the plastic opening of the deltoid muscle demonstrating the filling of the correct muscle 7
  • 8. Post procedure • The student will remove the needle quickly & dispose of needle in sharps box • The student will massage the area of injection • The student will assess site for any bleeding (Band-aid if small drop) • . 8
  • 9. • The student will assess Post Procedue tolerance of procedure from patient (SIM man will provide positive feedback if ALL steps were followed correctly and in proper sequence-if not-it will identify areas requiring improvement) • The student will clean up and through trash away • The student will remove gloves • The student will wash his hands • The student will document administration in the MAR 9
  • 10. Assessment & • The student will inquire Documentation if SIM man needs anything else (response will be no if all steps are followed) • The student will return within the hour to check patients response to medication • The student will document efficacy of intervention in the MAR 10
