Overview of PACE
(PACE) is a Medicare and Medicaid program that
helps people meet their health care needs in the
community instead of going to a nursing home or
other care facility.
Currently operating out of 31 states
8. • Care and services in the home, the community,
and the PACE center.
• Have contracts with many specialists and other
providers in the community to make sure that you
get the care you need.
• Participants get most of their care from staff
employed by the PACE organization in the PACE
center.
• PACE centers meet state and federal safety
requirements.

Ronni Lyons
17. Jacquelynn brings more than 30 years
of experience to her role as center
director for InnovAge’s Germantown
LIFE center, having served seniors in
Philadelphia through the LIFE program,
affordable housing, adult day care,
skilled nursing, and senior centers.
Jacquelynn earned her master’s degree
in health administration from St.
Joseph’s University, and has a
bachelor’s degree from Temple
University in health management
studies. She is also a licensed nursing
home administrator in Pennsylvania.

Meet the Center Director-
Jackie Williams
18. A North Philadelphia native, Aja brings over 13
years of experience and compassion
in serving the community she grew up
in. From Olney (19120) area of Philadelphia, Aja
attended Philadelphia High School for Girls; following
a Bachelor’s of Science in Business Administration
majoring in Marketing and International Business from
Drexel University.
Business savvy with the focus on providing social service and giving
back to the community has always been important to Aja who
volunteered with the American Red Cross and organized
personal drives to give to the homeless during the holidays at a very
young age.
Aja is new to the InnovAge family but has been working as
PACE/LIFE Navigator for the past year, first joining the Mercy
LIFE team, later compelled to provide education about LIFE services
to those closer to home. Aja’s pass roles in Public Relations and
Business Development for various companies including medical
practices in Brooklyn, New York has always led her to focus on the
needs of the consumers giving a special kind of boldness of care and
concern for the individuals she provides service too.

Nearly 90 percent of seniors want to stay in their own homes as they age, often referred to as “aging in place.”
The U.S. Centers for Disease Control and Prevention defines aging in place as "the ability to live in one's own home and community safely, independently, and comfortably, regardless of age, income, or ability level."
A majority of aging adults want to stay at home, yet nursing home placement is frequently the only option to care for their complex medical condition. (Journal of the American Society on Aging, 2006)

LIFE (Living Independently For Elders) is an alternative option to nursing facilities that helps seniors age independently. InnovAge LIFE provides nearly all medical care and social support at a center nearby.

The Program of All-Inclusive Care for the Elderly (PACE), is a Medicare and Medicaid state option program for those who are eligible for nursing home-level care but can safely reside in the community. The program integrates both acute and long-term care, amounting to a continuum of both medical and social services. In addition to better clinical outcomes through shorter hospital stays, lower mortality rates, and better self-reported health and quality of life, costs for PACE enrollees are estimated to be 16 to 38 percent lower than Medicare fee-for-service costs for the elderly population and 5 to 15 percent lower than costs for comparable Medicaid beneficiaries.
Civic engagement and volunteering can reduce mortality; increase physical function, muscular strength, and levels of self-rated health; reduce symptoms of depression and pain; and increase life expectancy.
