Sustainable Models of Care Oncology Homecare – Patients assessed via telephone and receive treatment at home instead of in hospital – Patient waiting time reduced significantly – Patient satisfaction increased – Reduction in clinic and day unit congestion, freeing up more time for care – Winner of HSJ Award 2013

Outpatient & Home Parenteral Antibiotic Therapy (OHPAT) • Clinical – Almost 300 emergency admissions avoided – Over 4,000 bed days saved – Reduced risk of cross infection – No adverse outcomes (i.e. MRSA/C.Diff/readmissions/line infections/deaths) • Environmental – Over 420 tonnes of CO2e/yr saved through inpatient admission avoidance – Over 12,000 travel miles eliminated annually (almost 4 tonnes of CO2e/yr) • Social – Patient satisfaction: 100% • Financial – Over £150,000 saved (in avoided outpatient tariff cost) – Reduced cost of patient-related travel Sustainable Models of Care Highly Commended in HSJ Awards 2014

Sustainable Models of Care Haematology Extracorporeal Photopheresis • Ward 33 (FH) and Bright Red bought two ECP machines for bone marrow transplant patients at Freeman. • This has saved c.20 immunocompromised patients a year from travelling out of the region (250 mile round trip). • Improved patient outcomes, patient satisfaction, reduced patient travel cost and associated carbon emissions.
