Summary
The National Implementation & Dissemination for Chronic Disease Prevention Project brought
together the American Heart Association, the American Planning Association, the National WIC, the
Directors of Health Promotion and Education and the Society for Public Health Education. Their
mission is to implement, evaluate and disseminate evidence- and practice based community health
activities. Their goal is a five percent reduction death and disability from tobacco use, a three
percent reduction weight-related health issues, and a three percent reduction in rates of death and
disability due to diabetes, heart disease, and stroke. However, a framework was needed for the
collaborative and strategic partnership.
Challenge
Bridging the missions and organizational competencies of five national organizations: the
American Heart Association, the American Planning Association, the National WIC, the
Directors of Health Promotion and Education and the Society for Public Health Education.
The challenge was to combine the complementary strengths of each to provide a platform
to launch their collective commitment to chronic disease prevention and health equity. Work
was required to address each organization’s policies and procedures of engaging multiple
audience sectors to help increase awareness and understanding of community health
chronic disease prevention strategies among national stakeholders, general and priority
publics.
2. SOPHE assisted in coordinating the National Collaborative Strategy Meeting to further
enhance the partnership focused on building the capacity to assess the need,
implementation of evidence-based policy, systems and environmental change approaches
and communicating shared messages. A technical assistance framework was developed for
use by all five organizations in the partnership.
The National Collaborative Strategy Partnership established the technical assistance
framework which will maintain individual project and organizational goals and emphasize
a shared commitment to chronic disease prevention. The development and sustainability
of the framework will outlive the collaborative project period.

The national organizations convened face-to-face meetings to strategize
ways to maximize the efforts of the sub-recipients. The group, Community
Initiative, facilitated the discussion on understanding the strengths and
opportunities of each organization in an overall effort to effectively increase
technical assistance to sub-recipients. Combining the knowledge and
networks of each national organization will raise awareness across sectors,
leverage existing resources and generate new connections.
