PROCEDURES
Basic techniques: In the course of my training I have gained confidence in
temporary cardiac pacemaker insertion, pericardial aspiration and Intra Aortic Balloon
Pump (IABP) insertion.
Diagnostic catheterization: During my training in the Catheterization Laboratory I
gained confidence and wide experience in performing right and left cardiac
catheterization, coronary angiography and peripheral vascular catheterization.
Performance of a large number of diagnostic catheterizations, including use of radial
and brachial artery access. Conversion to radial artery as the preferred access site
since 2013.
Coronary interventions: Performance of more than 300 interventions, while
training in Milan and around 80-100 procedures/year since, for a variety of coronary
lesions including complex ones:
coronary bifurcations, with stenting of the main branch and balloon angioplasty
for the side branch or double stenting techniques: modified T stenting and crushing
technique
in-stent restenosis, with the use of conventional and cutting balloon angioplasty,
occasionally followed by beta intracoronary brachytherapy
calcified lesions, with the use of cutting balloon or FX Minirail balloon angioplasty
and rotablator use
diffuse disease & small vessel disease, with use of intravascular ultrasound and
intracoronary pressure measurements for definition of the appropriate treatment
strategy
unprotected left main stenosis, ostium, shaft and bifurcation, while working in
Milan and at my personal practice in Euroclinic Hospital after the results of SYNTAX
Trial.
Radial artery cannulation is the preferred access site for coronary interventions.
Working with Dr A. Colombo (2000-2003) I also had the opportunity to use a lot
of different devices, many used in our centre as initial evaluation in clinical trials:
the Galileo intracoronary beta-brachytherapy catheter,
different atherectomy devices as the “Fox-Hollow” catheter,
the rotablator catheter,
the cutting balloon coronary catheter,
the Angioguard and the Percuserge protection devices,
the “Rescue” thrombus aspiration coronary system,
the “Volcano” and the “Epiphany” thermography catheters,
laser catheters and
the “FrontRunner” device for chronic total coronary occlusions.
7. 7
Non Coronary interventions:
At Iatrikon Heart Center we have organized a Trans Catheter Aortic Valve
implantation team and we have successfully treated the first 3 patients.
I have also participated in a discrete number of peripheral vascular interventions
while working at San Raffaele hospital (carotid arteries, renal arteries, iliac and
femoral arteries).
Intravascular ultrasound: I also had the opportunity to gain wide experience in
performing and evaluating intravascular ultrasound (IVUS) examination
- either for diagnostic purposes:
Cases where the angiography is inconclusive as regards the significance of
stenosis, especially in left main coronary artery disease and ostial disease
- or as a tool to help determine the treatment strategy during coronary
interventions:
During implantation of bare and drug-eluting stents, in cases of in-stent restenosis
and in cases of diffuse or small vessel disease.
RESEARCH
Participation in research work organized by the department during my cardiology
training at Hippokration hospital, mainly work with intravascular ultrasound (IVUS).
Participation in clinical research in San Raffaele & Centro Cuore Columbus:
quantitative coronary angiography (QCA) and intravascular ultrasound (IVUS)
measurements, clinical and angiographic follow-up of patients, as well as data
analysis and presentation of the results at international meetings.
Participation in the conduction of multi-center trials since September 2002, due
to my duties in Mediolanum Cardiovascular Research (MCR) center. During this time
I gained experience with the design and performance of multicenter clinical trials, as
well as evaluation and presentation of the results. Amongst others:
Clinical Project Leader in CIRIUS Bifurcation Trial (the first study using the
CYPHERTM stent for coronary bifurcations) and co-ordination of IVUS subanalysis of
the study.
The study has been conducted in 5 centres (Centro Cuore Columbus in Milan, Lenox-
Hill Hospital in New York, ICPS centre in Massy, Paris, Mayo Clinic in Rochester,
Minessota and University of Erlangen, Germany) with Dr Antonio Colombo as
primary investigator.
Conceptualization and structure of the protocol of the CACTUS (Coronary
Bifurcations: Application of the Crushing Technique Using Sirolimus-Eluting
Stents) Trial organized by the MCR aiming to define best available technique for
coronary artery bifurcations treatment.
IVUS sub-analysis from the Rescut (REStenosis CUTting balloon evaluation)
study, a multicenter trial that compared cutting balloon to conventional balloon
angioplasty in the treatment of in-stent restenosis (the clinical results were presented
during the Transcatheter Cardiovascular Therapeutics 2002 - late breaking clinical
trial).
