This talent release form authorizes Tina Hartnell, a producer, to use the person's appearance in a public awareness advert, radio advert and pop up advert. It allows the producer to record the person's voice, photograph and likeness for the advertisements. The producer will own full rights to the footage and can use the person's name and likeness to promote the ads and producer's services without restrictions. The person will not be compensated and understands the master recordings belong to the producer.

I understand that I will receive no compensation for my appearance in the above
advertisements. The producer will have the full ownership of all footage. I am giving the
producer the right to use my name, likeness and biographical material to publicize the
advertisement and services of the producer.
The producer may:
1. Record my voice and photograph me and likeness for the purposes stated above
production whether by film, videotape, digitally, by magnetic tape or otherwise.
2. Make copies of all of the footage gathered stated above.
3. Use my name and likeness for the purpose of education, promotion or advertising of the
sale or trading in the photographs, recording and any copies that may be made.
I also understand that the master tape remains the property of the producer and that there
will be no restrictions on the number of times the film may be copied or the number of times
that my name or likeness is used.
