This talent release form authorizes the producers to videotape Lauren Rosenfeld, Millie West, Ben Karrass, and Alex Pereira on an unspecified date and gives the producers full rights to the videotape, including reproducing, copying, exhibiting, publishing, and distributing the video. It releases the producers and their affiliates from any legal claims related to the video. The talent and producers listed must sign to agree to these terms.

I hereby assign and authorise the producer(s) ___________________________ the
right (all rights) to such a videotape. I also authorise the said producer, without
limitation, the right to reproduce, copy, exhibit, publish or distribute any such
videotape, and waive all rights or claims I may have against your organisation
and/or any of its affiliates, subsidiaries, or assignees, other than as stated in this
agreement.
