3. 8 Rights of Drug
Administration
Instead of the 6 rights of medication
administration.
4. Right patient
• Ask patient to identify himself/herself.
• Check the name on the medication administration
record (MAR) with what the patient says.
• Use at least 2 forms of identifiers,
• When available, use technology (for example, bar-
code system).
• In long term facility medication aides use the
picture on the MAR

• List all the different ways to identify the right patients in your facility?
• How would you identify a patient diagnosed with dementia?

6. Right Medication
• Right medication
• Check the medication label.
• Check the medication administration record (MAR)
• Compare the drug label with the MAR three times.
• Before removing the med from storage, before preparing and after preparing.
• Some drug names look and sound similar so make sure you have the correct drug
7. Right Dose
• Right dose
• Check the drug label against the MAR. three times.
• Confirm appropriateness of the dose using a current drug reference.
• Use the appropriate measuring device for liquid medication.
• Read level of liquid medication from the bottom of the meniscus or the
curve of the liquid surface. Pour medicine at eye level, and pour away from
drug label.
8. Right Route
• Right route; this is the way the medication is introduced into the body.
• Again, check the MAR, and drug label for the route ordered.
• Confirm that the patient can take or receive the medication by the ordered route.

• Medication aides can administer medication these routes except; PO, R, OS, OU,
AU, SUB Q, IM, NGT, Bucc, IV?

9. RightTime
• Check the frequency of the ordered medication.
• Verify if medication is to be administered before or after meals, at bedtime,
or before breakfast.
• Medications for seizure and Parkinson’s disease should be given at precisely
the scheduled time.
• Double-check that you are giving the ordered dose at the correct time.
• Confirm when the last dose was given especially for PRN orders.
10. Right documentation
• Document administration AFTER giving the ordered medication.
• Chart the time, route, and any other specific information as necessary for
PRN meds.
• For PRN you should document the vital sign parameters such as B/P, Pulse,
respiratory rate as well as pain levels.
• Documentation on the computer must be timely to avoid being written up
by your supervisor .
11. Right Reason
• Confirm the rationale for the ordered medication.
• What is the patient’s diagnosis?
• Why is he/she taking this medication?
• Medication aides should be able to match the
medication ordered with the patient’s diagnosis.
• The facility would have to Revisit the reasons for
long-term medication use.
12. Right Reason
Question:The medication clonidine is prescribed for the following conditions, select all that

13. Right Response
• Make sure that the drug led to the desired
effect. If an antihypertensive was given,
has his/her blood pressure improved?
Does the patient verbalize improvement
in depression while on an antidepressant?
• Report side effects as well as adverse drug
reactions.
• Be sure to document your monitoring of
the patient as per facility’s policy
