Senior Care Options

Braintree - South Shore Elder Services,
Inc. (SSES) is made up of many depart-
ment’s providing services and resources to
our consumers. One of the departments at
SSES is Senior Care Options (SCO). SCO
“is a partnership between MassHealth and
Medicare that provides an integrated and
complete package of health care and so-
cial services for low-income seniors.
One of the main goals of Senior Care
Options is to help seniors stay healthy,
in control of their lives and out of nurs-
ing homes” (Massresources.org). Two of
the eligibility requirements are consumers
must be at least 65 years of age and be eli-
gible for MassHealth Standard.
Where there are some gaps in MassHealth
Standard and Medicare benefits, the SCO
programs offer benefits with many of
those gaps filled.
At SSES, we have three different SCO pro-
grams at this time: Senior Whole Health
(SWH), United Healthcare and Navicare.
All three programs provide many bene-
fits to our members, helping elders stay
healthy in the community.
A few of the many benefits provided by
these SCO programs are primary care,
emergency care, x-rays, lab tests, prescrip-
tion drugs, vision care, dental care, mental
health, services at home to assist with daily
living needs and hospitalizations.
SCO is an option for elders in long-term
care facilities also. Elder’s who meet the el-
igibility requirements and reside in a long-
term care facility are provided with the
care they need in their living environment.
These elders receive many of the same
benefits as elders residing in the commu-
nity would.
Each consumer enrolled in the SCO pro-
gram is assigned to work with a Geriatric
Support Specialist Coordinator (GSSC).
The GSSC collaborates with the consumer
and the SCO staff to coordinate an appro-
priate service plan to effectively address
each consumer’s needs. The GSSC also
advocates on the behalf of the consum-
er and educates them about the services
available in the community. The GSSC has
contact with the consumer on a quarterly
basis.
