3. Procedure of Health Assessment:
• Before health Assessment, the nurse (we)
should conduct a classroom health lecture
and inform the children on what they are to
do during the assessment,
• Three to five children at a time should be in
waiting for the assessment.
4. Procedure of Health Assessment:
• We should wash our hands with soap and
water before the start of the assessment.
• Client to be assessed one by one.
• Complete inspection should be made from
head to foot for cleanliness, skin diseases
and signs of abnormal condition.
5. Procedure of Health Assessment:
• Stethoscope must be used for the heart
and lung assessment.
• Findings should be recorded during the
assessment.
6. Steps in the conduct of Health
Assessment:
1st Arms, hands, and finger nails: Children should be
told to roll their sleeves, extend their arms, show their
hands one side first, then the other and spread their
fingers.
2nd Eyes: The child should pull down lower lids using
his index finger placed under them, and then tell them
to look up.
3rd Nose: The child should place his second finger on
the tip of his nose and pull up his nose and extend his
head backward.
7. Steps in the conduct of Health
Assessment:
4th mouth: The child should be told to open his
mouth and say “ah” to show his throat.
5th Ears: The child should be requested to push
back his hair behind his ear and pull the outer ear
up, slightly backward and the forward.
6th Neck and Chest: The neck should be
examined. Chest and back should be auscultated.
8. Steps in the conduct of Health
Assessment:
• 7th Hair: The pupil should be asked to run his fingers
through his/her hair several times. He may show the
back of the neck by pushing the hair up.
• 8th Feet and Legs: The girls should pull up her dress
and the boy, his trousers to their knees. Observations
may also be done while the children are marching in
and out of the room. Children may also sit sideward
so as to have their legs extended out in the aisle.
• 9th General Appearance: The child’s general
appearance should be observed.
