1. BY CHEQUE - Please complete and detach the slip below and forward your cheque for the appropriate fee made payable to
“Little Kickers” to the address at the top of this form. You will be contacted regarding your startdate/availability upon receipt.
2. BY BANK TRANSFER - Please complete and detach the slip below and forward to the address above. We can accept a payment
by bank transfer once we have received your completed enrolment form and your start date has been confirmed, and will happy to
provide you with our account details at that point.
3. ONLINE PAYMENT - If you prefer to complete a paperless enrolment for your child’s course online, and pay us using your debit
or credit card, you can complete the whole process on our website www.littlekickers.co.uk Sign Up page.
4. BY TELEPHONE - Please complete and detach the slip below. We are able to accept payment for your new course over the
telephone via debit/credit card upon receipt of your completed signed enrolment form, and when a start date has been confirmed
