PPS Spotlight:
Adirondack Health Institute (AHI) PPS Medical Village
Transformation
Reducing excess bed capacity and repurposing inpatient hospital infrastructure
into “medical villages” is underway in the Adirondack region for three hospital
organizations.
There are various community needs identified as primary drivers of emergency
department utilization in the region, including behavioral health, mental health and
substance abuse. In the AHI PPS’ service area, the conditions causing most inpatient
admissions for Medicaid members are mental diseases and disorders and diseases and
disorders of the cardiovascular system. These conditions account for 64 percent of
admissions when grouped by major diagnostic categories (MDC). Behavioral health
conditions (mental diseases and substance abuse disorders) account for 49 percent of
admissions. Three new and distinct outpatient centers will provide services for the high-
need, high-risk patient population through infrastructure modifications and renovations.
The University of Vermont Health Network - Champlain Valley Physicians Hospital
(UVMHN-CVPH) is establishing a medical village focusing on the needs of seriously ill
adults needing inpatient and outpatient behavioral health care services. The project
involves renovating and repurposing unneeded inpatient hospital infrastructure to create
an integrated outpatient Behavioral Health Transition Services Center. The center will aid
patients during and after discharge to assure a smooth transition into the community,
assist in the coordination of care, and establish systems for “warm” handoffs from the
acute phase of care to a less-costly, non-hospital-based setting.
The Glens Falls Hospital (GFH) medical village project entails providing readily-
accessible behavioral health crisis services to adults and adolescents, supporting a rapid
de-escalation of the crisis facilitated by the appropriate level of service and providers.
Plans are to renovate existing hospital space to create a new Crisis Stabilization Unit. The
unit will be a single source of specialty care management for complex adult and pediatric
patient conditions, with clear linkages to GFH hospital services, the Emergency Care
Center, Health Home care coordination, and community-based organizations.
Inter-Lakes Health (Moses Ludington Hospital), with the assistance of a number of
partner organizations, is developing a medical village that will offer a variety of robust,
complementary health-related services for the convenience of patients on their main
Ticonderoga campus. Among other transformed hospital services operated by University
of Vermont Health Network - Elizabethtown
Community Hospital (UVMHN-ECH), the medical
village will offer high-quality, on-site primary care
provided by Hudson Headwaters Health Network
(HHHN), emergency care, long-term care operated
by Post Acute Partners under its Elderwood brand
of Nursing Homes, laboratory services, radiology
testing and existing senior housing under Lord
Howe Estates and Moses Circle Senior Housing.
