We don't let pilots or drivers get behind wheels if they are sleep deprived. But for a long
time, long hours seemed to be the only way to train young resident doctors, who had no
option but to work even if they swayed on their feet due to lack of sleep. “Hundred-hour
workweeks, complete with regular all-night shifts, were standard,“ says Jordan Weissmann
in Slate.com.
Eventually, doctors and the general public started realising the danger of sleeplessness
among residents and how it could risk the lives of patients. In the US and elsewhere, there
are rules limiting the hours young physicians can work.
However, the regulations don't seem to benefit patients. In some cases, they even seem to
be backfiring.
The effort to get young doctors more sleep dates back to the 1984 case of Libby Zion, an
18-year-old woman who died at a New York hospital hours after being admitted with a
103-degree fever. Her father and New York Daily News columnist Sidney Zion drew
attention to the fact that the young trainee who had treated his child had been on call for
almost 24 hours. Eventually, New York became the first state to limit residents' hours.

But studies have found that the new schedule isn't improving patient
outcomes. Reason? The new rules prevented doctors from overseeing their
patients' care from start to finish. “Studies comparing teaching and
nonteaching hospitals have found that the reforms were associated with an
increased risk of complications for patients undergoing neurosurgery, spinal
surgery, and coronary artery bypass grafts,“ writes Weissmann.
The rules are a problem for surgeons especially , says Dr Karl Bilimoria, a
professor at Northwestern University's Feinberg School of Medicine, because
young doctors find themselves forced to hand off patients in the middle of
urgent situations.
In the meantime, there are some signs that interns themselves may not be
benefiting much. They are no less likely to experience depression. Ironically
enough, they also didn't seem to get much, if any, more sleep, writes
Weissmann.
