APPLICATION OF COMPUTER IN
HOSPITAL PHARMACY
INTRODUCTION
Pharmacy field plays an essential role in patient health care. The field
of pharmacy is massively benefitted by usage of computers.
Currently computers used in pharmacy field for their accuracy,
multitasking, huge data storage, reduction in time, manpower and for
various purpose.
APPLICATION OF COMPUTER:
Maintenance of patient records
Maintenance of drug records
Purchase and Inventory control record
Therapeutic drug monitoring
Drug Information retrieval
Drug dispensing
Avoiding medication error
Record relating to drugs in hospital formulary
MAINTENANCE OF PATIENT RECORDS IN HOSPITAL
PHARMACY
Patient record maintenance is important job in hospitals but with the
support of computers, data can be preserved easily and also updated
time to time.
Because the work of hospital pharmacy revolves around patient care
in the hospital the patient record should consist of;
i) Personal information of the patients
ii) Record relating to individual drug prescribe to the drug
administration, change in dose of drug, changes in drug selection for
the patients.
iii) Record relating to charges or bill to the patient for charge floor
drugs.
iv) Entry of medication errors
v) Patient medication profile

Date of Admission
Ward number
Bed number
MAINTENANCE OF RECORDS RELATED TO PATIENT
MONITORING
i) Record relating to different physiological parameters like blood
pressure, temperature, pulse rate, blood oxygen level etc, are
monitored during therapy.
ii) Record relating to ADR if seen.
iii) Record relating to DUR in the patient.
iv) Date on known drug interaction.
PURCHASE AND INVENTORY CONTROL RECORD
Computer can be used efficiently for achieving inventory control.
Hospital pharmacist initially follow periodic control system & then
convert to perpetual system.
As maintenance of perpetual inventory control, maintaining the
inventory records, calculation of inventory, automatic updating of
the price, evaluation of demand.
Periodic inventory control system:
• In periodic inventory system, inventory levels are manually checked
and entered into the database.
• The amount of inventory on hand is compared with minimum &
maximum stock is entered.
• Computer will help in placement of order to each supplier by
determining usual or minimal order quantities for supplier.
Perpetual inventory control system
• In perpetual system computer maintains the running balance of the entire drug
in stock.
• All drugs are entered into the database when they are received in the pharmacy,
they are added to the initial stock so as to get the current stock levels of each drug.
• The quantities of all drugs leaving the pharmacy are subtracted from the inventory
balance. This will occur whenever the drug order of the patient is processed.
• Computer prepares a list of drugs to be ordered. It will also prepare a purchase
order forwarded to the supplier.
THERAPEUTIC DRUG MONITORING
To maximize the therapeutic action of medicines, avoiding any toxic
effects pharmacist has started taking assistance of some
pharmacokinetic & non-pharmacokinetic parameters.
Therapeutic drug monitoring (TDM) is most important in geriatric,
pediatric patients and drugs having high tendency for interactions.
TDM aims to improve treatments by personalizing dosage treatments.
Clinical pharmacist may uses computers for therapeutic drug
monitoring; in which drugs are very potent and have a very
narrow therapeutic index like cardiac glycosides,
antiepileptics.
Computer program are intended to calculate drug dosage to suit
individual patients need.
a) Pharmacokinetic applications
Estimation of pharmacokinetic parameters includes statistical calculation &
graphical interpretations.
It needs frequent blood samples to be collected.
By using the pharmacokinetic parameters, drug dose, time of administration
can be predicted by computers.
Example:
Computer programs like WinNONLIN, USC Pack softwares are used to
predict the pharmacokinetic parameters.
b) Non pharmacokinetic applications
It includes detection of drug-drug, drug- laboratory, drug allergy, ADR &
drug-disease interaction detected by computer software.
The computer system enables detection of potential ADR and notification to
the patient’s physician prior to administration of the prescribed drug.
Example: Software used for drug interaction screening are;
MEDIPHOR (Monitoring & Evaluation of Drug Interactions by a Pharmacy
Oriented Reporting),
PAD (Pharmacy Automated Drug Interaction Screening).
DRUG INFORMATION RETRIEVAL:
A broad search of the drug information is required for the pharmacist
so as to satisfy the queries about pharmacology, toxicology, drug
interaction, adverse drug reaction etc.
Search of drug information can be simplified by the use of computers.
In 1964, National library of medicine has made a computerized
biomedical information retrieval system, MEDLARS (Medical
Literature Analysis and Retrieval System).
In 1971, they developed a fast-working system MEDLINE (Medlars On-
Line)/ PubMed. It provides free online biomedical literature database.
The computerized information retrieval has following advantages over
the manual search,
 Time saving
 It is more thorough & timelier than manual search.
DRUG DISPENSING
Computer is also being effective for drug distribution.
The computer system is accepted to decrease the time spent by
pharmacists on routine distribution tasks.
The system appears to improve efficiency of pharmacist accuracy,
control of drugs and capabilities for patient monitoring.
Computerization of narcotic drugs and controlled substances may help
in avoiding excessive supplies and unauthorized distribution of such
drugs.
The specific identification codes are given to authorized individual.
Hence authorized individual can directly link through computer to find
dispensing record.
Eg: DAWN (Drug Abuse Warning Network) program is used for
dispensing controlled substances.
AVOIDING MEDICATION ERROR
Computers play a vital role in preventing medication errors.
Computerized Physician Order Entry (CPOE) has been shown to decrease
medication errors by 55-80%.
CPOE is the process which contains placing clinical order like prescription,
laboratory test request and result using a computerized system by health
care providers.
Computer applications allow pharmacists to easily analyze orders and
detect DRP (Drug Related Problems).
ELECTRONIC HEALTH RECORD
The EHR systems are basically designed to collect, store,
manage and display health care related records entered by
healthcare provider.
They store the past health history, radiology reports,
treatments, lab tests, treatments given, allergies,
immunization, physicians recommendations and other
relevant data that describe the health status of patients.
These computerized records have been broadly used by
clinicians, paramedical staff, patients, and health insurance
companies for the purpose of generating, managing, and
retrieving the health information of patients everywhere.
Definition: EHR systems are computer-based application used to
convert all the information found in paper based medical records
to a computer-based system.
Examples: eClinical Works, Allscripts
Reasons for collecting medical record:
Traditional health care systems face many challenges include
massive work done manually on papers.
The increased size of the database is another problem, and hard to
update it.
In addition, there is a need for huge space to store these paper
files.
Thus, the rapid development of information technology
changed the conventional healthcare system and electronic
health systems started to adopt on a larger scale.
Healthcare providers believe these electronic systems as a
key for solving the difficulties of traditional systems.
Advantages of EHR
Provides a summary of the various healthcare events in the life of
a person.
Collects all information from patient records which helps to
provide evidence-based treatment.
Patient information can be accessed from any place.
Data can be easily captured or transferred from one device to
other.
Errors can be reduced
Reduces the potential risk of miscommunication between system
users.
EHR systems can be connected directly to medical devices which
triggers alert upon a significant change in a patient health.
The EHR systems maintain patients information in more secured
manner compare to paper based systems.
Increases accuracy and faster diagnosis that determines the most
suitable medications for diseases, and monitoring the patient and
health in the future.
Avoid repeating unnecessary investigations
Useful for tracking and monitoring diseases
Helps in selecting the most efficient paths of treatments.
Provides effective, safe and efficient healthcare services, and
decreases cost of healthcare.

  • 2. INTRODUCTION Pharmacy field plays an essential role in patient health care. The field of pharmacy is massively benefitted by usage of computers. Currently computers used in pharmacy field for their accuracy, multitasking, huge data storage, reduction in time, manpower and for various purpose.

  • 4. MAINTENANCE OF PATIENT RECORDS IN HOSPITAL PHARMACY Patient record maintenance is important job in hospitals but with the support of computers, data can be preserved easily and also updated time to time. Because the work of hospital pharmacy revolves around patient care in the hospital the patient record should consist of;
  • 5. i) Personal information of the patients ii) Record relating to individual drug prescribe to the drug administration, change in dose of drug, changes in drug selection for the patients. iii) Record relating to charges or bill to the patient for charge floor drugs. iv) Entry of medication errors v) Patient medication profile
  • 7. MAINTENANCE OF RECORDS RELATED TO PATIENT MONITORING i) Record relating to different physiological parameters like blood pressure, temperature, pulse rate, blood oxygen level etc, are monitored during therapy. ii) Record relating to ADR if seen. iii) Record relating to DUR in the patient. iv) Date on known drug interaction.
  • 8. PURCHASE AND INVENTORY CONTROL RECORD Computer can be used efficiently for achieving inventory control. Hospital pharmacist initially follow periodic control system & then convert to perpetual system. As maintenance of perpetual inventory control, maintaining the inventory records, calculation of inventory, automatic updating of the price, evaluation of demand.
  • 9. Periodic inventory control system: • In periodic inventory system, inventory levels are manually checked and entered into the database. • The amount of inventory on hand is compared with minimum & maximum stock is entered. • Computer will help in placement of order to each supplier by determining usual or minimal order quantities for supplier.
  • 10. Perpetual inventory control system • In perpetual system computer maintains the running balance of the entire drug in stock. • All drugs are entered into the database when they are received in the pharmacy, they are added to the initial stock so as to get the current stock levels of each drug. • The quantities of all drugs leaving the pharmacy are subtracted from the inventory balance. This will occur whenever the drug order of the patient is processed. • Computer prepares a list of drugs to be ordered. It will also prepare a purchase order forwarded to the supplier.
  • 11. THERAPEUTIC DRUG MONITORING To maximize the therapeutic action of medicines, avoiding any toxic effects pharmacist has started taking assistance of some pharmacokinetic & non-pharmacokinetic parameters. Therapeutic drug monitoring (TDM) is most important in geriatric, pediatric patients and drugs having high tendency for interactions. TDM aims to improve treatments by personalizing dosage treatments.
  • 12. Clinical pharmacist may uses computers for therapeutic drug monitoring; in which drugs are very potent and have a very narrow therapeutic index like cardiac glycosides, antiepileptics. Computer program are intended to calculate drug dosage to suit individual patients need.
  • 13. a) Pharmacokinetic applications Estimation of pharmacokinetic parameters includes statistical calculation & graphical interpretations. It needs frequent blood samples to be collected. By using the pharmacokinetic parameters, drug dose, time of administration can be predicted by computers. Example: Computer programs like WinNONLIN, USC Pack softwares are used to predict the pharmacokinetic parameters.
  • 14. b) Non pharmacokinetic applications It includes detection of drug-drug, drug- laboratory, drug allergy, ADR & drug-disease interaction detected by computer software. The computer system enables detection of potential ADR and notification to the patient’s physician prior to administration of the prescribed drug. Example: Software used for drug interaction screening are; MEDIPHOR (Monitoring & Evaluation of Drug Interactions by a Pharmacy Oriented Reporting), PAD (Pharmacy Automated Drug Interaction Screening).

DRUG INFORMATION RETRIEVAL: A broad search of the drug information is required for the pharmacist so as to satisfy the queries about pharmacology, toxicology, drug interaction, adverse drug reaction etc. Search of drug information can be simplified by the use of computers. In 1964, National library of medicine has made a computerized biomedical information retrieval system, MEDLARS (Medical Literature Analysis and Retrieval System).
  • 16. In 1971, they developed a fast-working system MEDLINE (Medlars On- Line)/ PubMed. It provides free online biomedical literature database. The computerized information retrieval has following advantages over the manual search,  Time saving  It is more thorough & timelier than manual search.
  • 17. DRUG DISPENSING Computer is also being effective for drug distribution. The computer system is accepted to decrease the time spent by pharmacists on routine distribution tasks. The system appears to improve efficiency of pharmacist accuracy, control of drugs and capabilities for patient monitoring.
  • 18. Computerization of narcotic drugs and controlled substances may help in avoiding excessive supplies and unauthorized distribution of such drugs. The specific identification codes are given to authorized individual. Hence authorized individual can directly link through computer to find dispensing record. Eg: DAWN (Drug Abuse Warning Network) program is used for dispensing controlled substances.
  • 19. AVOIDING MEDICATION ERROR Computers play a vital role in preventing medication errors. Computerized Physician Order Entry (CPOE) has been shown to decrease medication errors by 55-80%. CPOE is the process which contains placing clinical order like prescription, laboratory test request and result using a computerized system by health care providers. Computer applications allow pharmacists to easily analyze orders and detect DRP (Drug Related Problems).

ELECTRONIC HEALTH RECORD The EHR systems are basically designed to collect, store, manage and display health care related records entered by healthcare provider. They store the past health history, radiology reports, treatments, lab tests, treatments given, allergies, immunization, physicians recommendations and other relevant data that describe the health status of patients.
  • 21. These computerized records have been broadly used by clinicians, paramedical staff, patients, and health insurance companies for the purpose of generating, managing, and retrieving the health information of patients everywhere. Definition: EHR systems are computer-based application used to convert all the information found in paper based medical records to a computer-based system. Examples: eClinical Works, Allscripts
