Maryland-Based Aledade Creating Primary Care Physician-Led ACO in Central Florida
New organization will use cutting-edge technology, big data, and analytics to improve patient
health, lower costs, and provide individually-tailored care.
Bethesda, MD - Physicians in the Sunshine State will soon have a new partner to keep their
patients healthy. Maryland-based Aledade, Inc. has announced the formation of a new
Accountable Care Organization (ACO) led by independent primary care physicians in Central
Florida. The organization will be the first physician-led ACO to receive admission, discharge,
and transfer notifications for its patients from Florida’s Health Information Exchange. Aledade
will partner with Clermont-based Primary Partners, LLC to provide on-the-ground practice
support to affiliated physicians.
Aledade partners with independent doctors across the United States to create and run ACOs -
networks of physicians who band together to deliver coordinated care. They operate under a
payment structure that rewards positive health outcomes for patients, instead of the volume of
health care services delivered. The new ACO will be managed by Kelly Conroy, Executive
Director of Aledade’s Florida division.

Aledade will work with physicians in central Florida to operate a Medicare Shared Savings
Program ACO, led completely by independent primary care doctors. Aledade ACOs focus on
2. delivering the highest-quality care to patients, with an emphasis on preventative care, high-value
referral networks, big data, and high-level analysis of patient claims and electronic health record
(EHR) data to identify patients who need help, before serious medical problems arise.
Aledade has already received commitments from 17 leading physicians in Florida, covering
6,300 Medicare beneficiaries, and will continue to recruit additional top doctors in the state
throughout the year. The Florida-based Aledade ACO will launch in January 2016. Several
recruited physicians have participated in ACOs previously, but joined the Aledade organization
because of the company’s previous success, its partnership arrangement with independent
doctors, and its commitment to continual, on-the-ground support of physician partners.

Kelly Conroy previously directed the Palm Beach Accountable Care Organization, one of the
earliest physician-led organizations in Medicare’s Shared Savings Program. The Palm Beach
ACO included 200 doctors covering 28,000 Medicare patients, and achieved $39 millions in
Medicare savings in its first year. Ms. Conroy has also advised the Centers for Medicare and
Medicaid Services (CMS) Learning Network on ways to improve the ACO program, and worked
to consult with physician-led groups on how to form and operate their own ACOs.
Under Dr. Mostashari’s and Ms. Conroy’s leadership, Aledade will bring regulatory expertise to
Florida physicians, ensuring all participating doctors meet necessary ACO and meaningful use
standards. Internally, the ACO will establish quality benchmarks and a physician-led governing
board to develop and share best practices for patient outreach, engagement, and treatment.
