2. Community Health
Workers Certificate
Community Health Care Workers (CHW) are a
vital link to promoting health and wellness
throughout communities and to lowering health
care costs. Certified Community Health Workers
act as part of a team within public-health, hospital,
social-service and other community support
organizations to promote health and wellness, and
provide support and services as directed by the
organization.
Core Services
CHW promotes public health and wellness to indi-
viduals and families in their communities through
“outreach, education, referral and follow-ups,
case management, advocacy and home-visiting
services… basic health education and referrals for
a wide range of services, and to provide support
and assistance in navigating the health and social
services system.”
The certificate will open up doors for employment
in many local organizations. Instructors are practi-
tioners, health educators, and experts who work in
the field.
Coursework
The certification can be obtained after successful
completion of:
• 96 hours of classroom training broken down
in four consecutive courses
• 96 hours of internship with area hospitals,
medical centers, Department of Health, other
health care organizations, or community
organizations
• Classes begin on January 13 and will end in May
• Internship will begin on or about June 1, 2015

Free Training
SCCC has been awarded a grant to train Community
Health Workers at no cost to participants who meet
the eligibility requirements:
• Household income requirements (200% of
federal poverty guidelines) or receives
Temporary Assistance for Needy Families (TANF)
• High school diploma or equivalency
• Prior experience in the field of health care (CNA,
HHA, PCA, EMT) or medical science OR
prior experience in community engagement,
either paid or volunteer demonstrated by
confirmation by sponsoring organization
• Immunization records and proof of a flu shot
• Passing of a criminal background check
• After the preliminary intake, there will be a
selection interview
In addition, all books and CDTA passes are provided
for each course.
