A 23-year-old female triathlete presented with varicose veins in her left calf and knee for 5 years. Physical exam found varicose veins in her left calf and knee up to 6 mm in size, as well as mild swelling in her left calf and thigh. Duplex ultrasound found incompetence in her common femoral vein and varicose veins in her greater saphenous vein, but no deep vein thrombosis. Angioplasty and stenting of her left common iliac vein improved her symptoms and reduced her varicose vein size. Six months later, she underwent ultrasound-guided foam sclerotherapy to further treat her varicose veins for cosmetic concerns.

Charting the menstrual flow on a calendar can be helpful to clarify normal versus abnormal cycles. Cycles that fall outside of the norm should be evaluated for underlying pathology. There are multiple causes for abnormal uterine bleeding in adolescents, the most likely cause is dysfunctional uterine bleeding (DUB) due to an immature hypothalamic-pituitary-ovarian (HPO) axis, causing an ovulatory cycles and irregular bleeding

combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,

modern vein care and its impact on patient outcomes. Dr. Elias has lectured, written and taught

Magazine and the Medical Editor of the American Venous Forum’s Newsletter, Vein Specialist. He
