4. Root Cause :
Action plan :
~ The stopper on the machine welding was loose due to nut didn’t lock and during
machine running the nut was move down (effect part over welding)
~ On that day line was problem on NC Riveting (diameter too small),
Final checker, too focus checking on diameter riveting and miss detect
to check of appearance Lens.
~ Lock of stopper in order that nut fixed (can’t up or down) and add
control point on Daily Machine Check List (before start Production test
manual should not gap and ensure the stopper have locking)
~ Briefing of final checker have to check all
appearance and share to all Leader about this issue NC Lens Melting.

because ratio of paint per tray average 42 gram
Modification size of equipment with reduce diameter 50%
each & ratio of paint should be average 30 gram
