The mission of The Center is to engage, empower, enrich, and advance the gay, lesbian,
bisexual and transgender (GLBT/LGBT) community of Colorado.

The GLBT Community Center of Colorado (The Center) has a dynamic thirty five year­long history of providing services and support to the 
LGBT community of Colorado. Throughout this time, The Center has continually strived to grow and change in order to best meet the needs of 
the LGBT community. In recent years, The Center has opened a new facility and has created newer programs, leading it into an innovative 
direction.  
 
Currently, The Center is in the process of creating a vision for the future which will be vibrant and effective for the LGBT community. This vision 
will also assist The Center in its efforts to further advance the LGBT movement. By providing your valuable opinions and feedback in this 
questionnaire, you will be helping The Center to build a future vision that is relevant to the Colorado LGBT community.

This Community Feedback Questionnaire is being conducted by The Center. The purpose of this questionnaire is to explore and describe the 
opinions and experiences of the Colorado LGBT community.  
 
The responses from this questionnaire will be used to help The Center in learning more about LGBT individuals living in Colorado and to 
inform a vision for The Center's 2012­2014 strategic plan. This vision for the future will then be shared with the community in early 2012.  
 
You are are invited to participate in this questionnaire because The Center is seeking your honest opinions and feedback.  
 
By choosing yes at the bottom of this page, you confirm that you understand and agree to all of the following statements: 
 
Participation in this questionnaire is completely voluntary; you may choose not to participate and you will not be penalized for doing so. If you 
decide to participate, you may withdraw your participation at any time without penalty.  
 
The questions in this questionnaire cover a variety of topics related to the LGBT community and The Center. Some questions may be 
considered personal or sensitive in nature. All efforts will be made to keep your responses confidential and no identifying personal information 
(i.e., name or email address) will be collected from the questionnaire.  
 
The process for participating in this questionnaire involves answering questions online in Survey Monkey. The questionnaire will take 
approximately twenty minutes to complete. You must complete the questionnaire in one sitting; you will not be able to return to it later on. 
 
The benefits of participating in this questionnaire include the opportunity to share your views and experiences with The Center and to therefore 
be involved in helping The Center to create a relevant, exciting plan for the future.  
 
Once you have completed the questionnaire, you will be given the opportunity to visit a separate website where you may enter yourself in a 
prize drawing. You may choose not to participate in this drawing.

Page 1 The mission of The Center is to engage, empower, enrich, and advance the gay, lesbian, bisexual and transgender (GLBT/LGBT) community of Colorado.   WELCOME TO THE CENTER'S 2011 COMMUNITY FEEDBACK QUESTIONNAIRE!    The GLBT Community Center of Colorado (The Center) has a dynamic thirty five year­long history of providing services and support to the  LGBT community of Colorado. Throughout this time, The Center has continually strived to grow and change in order to best meet the needs of  the LGBT community. In recent years, The Center has opened a new facility and has created newer programs, leading it into an innovative  direction.     Currently, The Center is in the process of creating a vision for the future which will be vibrant and effective for the LGBT community. This vision  will also assist The Center in its efforts to further advance the LGBT movement. By providing your valuable opinions and feedback in this  questionnaire, you will be helping The Center to build a future vision that is relevant to the Colorado LGBT community.     1. Introduction

Page 2 This Community Feedback Questionnaire is being conducted by The Center. The purpose of this questionnaire is to explore and describe the  opinions and experiences of the Colorado LGBT community.     The responses from this questionnaire will be used to help The Center in learning more about LGBT individuals living in Colorado and to  inform a vision for The Center's 2012­2014 strategic plan. This vision for the future will then be shared with the community in early 2012.     You are are invited to participate in this questionnaire because The Center is seeking your honest opinions and feedback.     By choosing yes at the bottom of this page, you confirm that you understand and agree to all of the following statements:    Participation in this questionnaire is completely voluntary; you may choose not to participate and you will not be penalized for doing so. If you  decide to participate, you may withdraw your participation at any time without penalty.     The questions in this questionnaire cover a variety of topics related to the LGBT community and The Center. Some questions may be  considered personal or sensitive in nature. All efforts will be made to keep your responses confidential and no identifying personal information  (i.e., name or email address) will be collected from the questionnaire.     The process for participating in this questionnaire involves answering questions online in Survey Monkey. The questionnaire will take  approximately twenty minutes to complete. You must complete the questionnaire in one sitting; you will not be able to return to it later on.    The benefits of participating in this questionnaire include the opportunity to share your views and experiences with The Center and to therefore  be involved in helping The Center to create a relevant, exciting plan for the future.     Once you have completed the questionnaire, you will be given the opportunity to visit a separate website where you may enter yourself in a  prize drawing. You may choose not to participate in this drawing.     If you have any questions or concerns regarding this questionnaire, you may contact Carlos Martinez or Heather Newhouse at The Center at  303­733­7743.    1.
