This document contains permission forms for two students, Cameron Frullo and Mason Dillnot, to be photographed for Jacob Grevatt's year 12 media portfolio. The forms give Jacob Grevatt permission to publicly display and share the images with others as part of his school assignment. The guardian of each student signs the form to provide consent.

explained the premise of the photoshoot and am
happy for Cameron Frullo to take part. I
understand, and give permission for said images
taken to be displayed publicly on Jacob Grevatt’s
blog and to be shared with students, teachers and
the examiner.
