1. Parental Consent Form
Please review the Official Contest Rules (located athttp://www.quinceanera.com/quinceanera-com-cover-girl-contest-2014/) and
this form. If you agree that your minor child may participate in XV Cover Girl Contest pursuant to the Official Contest Rules and
the terms of this consent form, then please fill in the blanks and sign below.

By signing this Parental Consent Form:
1. I hereby agree that my minor child may participate in the Cover Girl Contest being conducted by Quinceanera.com, and irrevocably grant my per-
mission and authorize Quinceanera.com to record, re-record, duplicate, touch up, edit, adapt, publish and commercialize recordings, photographic
images, audio tape, and videos as well as any digital recordings of my minor child including but not limited to her name, likeness, performance,
voice, and interviews throughout the world in perpetuity in all forms now known or hereinafter devised (“Media”) for the purposes of the Cover
Girl Contest (“Contest”) and for use in advertising, marketing, selling, publicizing and providing other promotional information concerning the
Contest. I further agree that the Contest, my minor child’s participation in the Contest, and all Contest elements, including but not limited to the
Media belong solely, entirely, and exclusively to Quinceanera.com. I understand and agree that Quinceanera.com and its affiliates, their agents, em-
ployees, licensees and assigns may exclusively duplicate, distribute, broadcast, exhibit, and otherwise publish information concerning the Contest,
its elements, and Media without limitation or restriction of any kind throughout the world in perpetuity as Quinceanera.com elects.
2. I understand and agree that neither my minor child nor I will acquire any ownership rights in the Contest, its elements, Media, nor will either of us
be entitled to receive any monetary compensation relating thereto. I understand and agree that Quinceanera.com is under no obligation whatsoever
to use my child’s name, likeness, performance, voice, interviews, or refer or otherwise mention my child’s participation in the Contest.
3. With appreciation of the privacy of my child and myself, I understand and agree that my child’s name and other personal information concerning
her that she or I provide in connection with her application for and participation in the Contest, including but not limited to the Contest Official
Rules which are incorporated herein, may be posted on the Quinceanera.com website or other public forum without any additional notice to or
permission from me.
4. I understand and agree that neither my minor child nor I will make any claim against Quinceanera.com and its affiliates, their agents, employees,
licensees, and assigns arising from my child’s application for or participation in the Contest, its elements, or the use of the Media. On behalf of my
minor child and myself, I forever release and agree to defend and hold harmless Quinceanera.com and its affiliates, their agents, employees, licens-
ees, and assigns from any and all claims and liabilities arising from my child’s application for and participation in the Contest, its elements, or the
use of the Media.
5. On behalf of my minor child and myself, I confirm my minor child and I are residents of the State of California. I understand that the terms and
conditions of this consent and release agreement shall be governed and construed by the laws of the State of California.
6. On behalf of my minor child and myself, I give my permission to Quinceanera.com for my child to participate in the Contest and accept any prizes
that may be awarded to her.
7. I represent and warrant that I am a legally competent adult either the parent of or lawfully appointed guardian for the minor child named above, and
that I have the lawful right to enter into this consent and release agreement on behalf of my minor child and myself.
8. Contest Official Rules. Preselected candidates will be notified by our staff and an online voting will be available for a week open to the public. Girls
with the highest scores will move on to the next round where an expert panel of judges will select the finalist. Finalists will have to show proof of
being 13-15 years old when they submitted their pictures.
The winner must be available for the interview and photo session immediately. The winner must be present at the Quinceanera.com Expo and
Fashion Show for which they were selected as the contest winner.
No need to buy to participate in the competition. Photos will become the property of Quinceanera.com.
The employees or immediate family members of Quinceanera.com and its affiliate companies are not eligible to participate in the contest. Contes-
tants authorize Quinceanera.com and its affiliates to use their names and photos in the future for promotional purposes.

9. I acknowledge and certify that by signing this consent and release agreement I have read, understood and voluntarily agree that I, my minor child,
and our respective heirs, representatives, and assigns will be bound by the terms and conditions of this consent and release agreement and the Con-
test Official Rules, which are incorporated herein. I agree to promptly provide all information concerning my child that is necessary to enter her in
the Contest and receive any prizes awarded.
