Health Insurance
All full-time graduate students must have health insurance.
If you do not have an alternative health insurance plan, you will be billed
for the Student Health Plan (SHIP) automatically.
Health Insurance Options:
• Required Student Health Insurance Plan for domestic students
(SHIP).
• Can be waived with alternative insurance.
• Required Student Health Insurance Plan for international students.
• Can be waived with alternative insurance.
• NYSHIP Student Employee Health Plan for TAs and GAs.
• UMR for RAs and Fellows.
All Students are

Required Student Health Insurance - SHIP
Domestic Plan International Plan
Fall/Winter(2018) - $1,411.25 Fall/Winter(2018) - $624.45
Spring/Summer(2019) - $1,944.25 Spring/Summer(2019) - $867.83
Fall Plan:
• Coverage from August 16th-January 15th.
Spring Plan:
• Coverage from January 16th-August 15th.
If you are part-time you can voluntarily enroll in SHIP.
• You must enroll by September 15th for fall semester, February 15th for spring and summer
semesters.
• You will not be automatically billed for SHIP if you do not voluntarily enroll.

• Must work at least ½ assistantship.
• Be paid at a rate of $4,293.00 annually.
• Be in active payroll status.
• Health insurance premium will be deducted from each
paycheck.
• International students are eligible to apply as soon as they
receive their Social Security Card.
For Domestic or
International (with J-1
or F-1 visas)
TAs/GAs.
• An enrollment form
• A copy of your Social Security card
• A copy of Birth Certificate or Passport
• Marriage Certificate (if filing spouse as a dependent)
• These documents are also required for each dependent you
want to include under this health insurance.
• TA/GAs will receive a notification/hold on their SOLAR account to
indicate what insurance they have. When the information is in the
system, Student Health Services will see it.
Not automatic;
must fill out form
and present valid
documentation to
Human Resources.
NYSHIP (TA, GA)
• Fill out a health insurance waiver at the start of the semester to not be billed for the regular
student health insurance.
• Must waive SHIP before September 15th of each academic year.
• Opportunity to waive only for spring/summer semester between January 1st and February
15th.
• Health insurance will start on the first day of your employment BUT you will not be officially
enrolled until the Health Services Office receives these documents.
• If your documents come in after the first day of employment, once they are received, your
health insurance will be retroactive back to the first day of employment.
• Best to apply early in the semester (before September 15th - fall or February 15th - spring).
• For summer coverage, you must be coming back in the fall to the same type of assistantship.
• Once you fill out the form and submit it, you will notice extra deductions taken out of your
paycheck to pay for the summer months.
• You do not need to fill out a form if you will be continuously working and receiving a
paycheck.

UMR (RA, Research fellows)
For Domestic or
International (with J-1 or

semester or the
equivalent in
months.
• Must be paid on a
bi-weekly basis
(not hourly).
• Must earn at least
165.12 each pay
period (salary of
$4,293.00).
• Must be in active

There are only two
opportunities to enroll in
the health insurance
plan.
• Within 30 days of
your appointment
date (deductions
will be taken
retroactively to
cover you from the
first day of
employment).
• During the annual
open enrollment
period in August of
each year.

UMR
Fellows are eligible
for UMR.
But because Fellows are not
technically employees of the
University, they must pay for
the total premium themselves.
During the summer
months that you are
not on active payroll
status, you must fill
out a continuation of
health insurance form
in spring in order to
continue coverage
over the summer.
In order to be eligible for
summer coverage, you
have to be:
1. Coming back in the fall
to the same type of
assistantship, verified by
their department.
2. Previously enrolled for
health insurance
coverage.Once you fill out the form
and submit it, you will
notice extra deductions
taken out of your paycheck
to pay for the summer
months.
You do not need to fill out a
form if you will be
continuously working and
receiving a paycheck.
Fellows who are not on
summer appointment do not
qualify for summer
coverage.

If you waive SHIP and lose
your alternative health

Services Office must be
notified immediately.
If you are a full-time
matriculated student,
they can put you on
SHIP retroactively.
Student Counseling and
Psychological Services:
• Located in the Student Health Center, also known as the Student Infirmary.
• On the north end of campus near the Union Building and Kevin P. Lavalle stadium.
• Check out hours here.
• Operates as a triage unit.
• When you walk in for the first time, you do not need an appointment.
• First appointment:
• Walk in during open hours.
• Fill out the required paperwork.
• Meet with a counselor that day.
• The triage counselor will talk to you about what is going on and discuss with you what you
are looking for.
• From there, the triage counselor will see what services you are eligible for and set
you up.
• You can visit counselling for any reason, not only in a crisis situation.
• Adjusting to campus, home-sickness, academic issues, social issues, etc.
Student Counseling and Psychological
Services: (cont.)
After the first appointment, the Center
has a variety of options for students
seeking help:
• Individual therapy
• Group therapy
• Consists of a socially related group.
• New groups forming each semester
• If there is a group of students that
have the same interest or issue that
they are dealing with, group
therapies can be created around
that issue.
• Mindfulness meditation
• Contact the counseling center
during the first few weeks of the
semester.
• Takes a few weeks to get the
meditation sessions going.
If you are not comfortable using the
campus counseling services, you can
be referred to an off-campus clinician
that is convenient for you.
• The Counseling Center has a
network of local and regional
clinicians for student referrals.
Counseling services are free and
confidential
• Not recorded on Academic
Transcript
• Parents and family are not
notified.
• There is a file that stays in the
Counseling Center.
• Release form must be signed in
order to show this file to a
professor or other staff member.

  • 3. Health Insurance All full-time graduate students must have health insurance. If you do not have an alternative health insurance plan, you will be billed for the Student Health Plan (SHIP) automatically.
  • 4. Health Insurance Options: • Required Student Health Insurance Plan for domestic students (SHIP). • Can be waived with alternative insurance. • Required Student Health Insurance Plan for international students. • Can be waived with alternative insurance. • NYSHIP Student Employee Health Plan for TAs and GAs. • UMR for RAs and Fellows.
  • 5. All Students are required to have health insurance. If you… …have alternative insurance (e.g. spouse, parents). …are a full-time student without employment or other insurance. …are a GA or TA. …are an RA/Fellow. You are eligible to waive the required student health insurance. You will be billed for the required student health insurance as part of your tuition. You are eligible for NYSHIP SHEP. You are eligible for UMR. Follow instructions on SOLAR, complete waiver before Day 15 of classes, EVERY semester. Once enrolled, you are eligible to waive required student health insurance. Once you find out which health insurance you are eligible for, click the links to skip ahead.
  • 6. Required Student Health Insurance - SHIP Domestic Plan International Plan Fall/Winter(2018) - $1,411.25 Fall/Winter(2018) - $624.45 Spring/Summer(2019) - $1,944.25 Spring/Summer(2019) - $867.83 Fall Plan: • Coverage from August 16th-January 15th. Spring Plan: • Coverage from January 16th-August 15th. If you are part-time you can voluntarily enroll in SHIP. • You must enroll by September 15th for fall semester, February 15th for spring and summer semesters. • You will not be automatically billed for SHIP if you do not voluntarily enroll.   • 7. NYSHIP (TA, GA) • Must work at least ½ assistantship. • Be paid at a rate of $4,293.00 annually. • Be in active payroll status. • Health insurance premium will be deducted from each paycheck. • International students are eligible to apply as soon as they receive their Social Security Card. For Domestic or International (with J-1 or F-1 visas) TAs/GAs. • An enrollment form • A copy of your Social Security card • A copy of Birth Certificate or Passport • Marriage Certificate (if filing spouse as a dependent) • These documents are also required for each dependent you want to include under this health insurance. • TA/GAs will receive a notification/hold on their SOLAR account to indicate what insurance they have. When the information is in the system, Student Health Services will see it. Not automatic; must fill out form and present valid documentation to Human Resources.
  • 8. NYSHIP (TA, GA) • Fill out a health insurance waiver at the start of the semester to not be billed for the regular student health insurance. • Must waive SHIP before September 15th of each academic year. • Opportunity to waive only for spring/summer semester between January 1st and February 15th. • Health insurance will start on the first day of your employment BUT you will not be officially enrolled until the Health Services Office receives these documents. • If your documents come in after the first day of employment, once they are received, your health insurance will be retroactive back to the first day of employment. • Best to apply early in the semester (before September 15th - fall or February 15th - spring). • For summer coverage, you must be coming back in the fall to the same type of assistantship. • Once you fill out the form and submit it, you will notice extra deductions taken out of your paycheck to pay for the summer months. • You do not need to fill out a form if you will be continuously working and receiving a paycheck. UMR (RA, Research fellows) For Domestic or International (with J-1 or F-1 visas) RAs. • Must be appointed for at least one semester or the equivalent in months. • Must be paid on a bi-weekly basis (not hourly). • Must earn at least 165.12 each pay period (salary of $4,293.00). • Must be in active payroll status. There are only two opportunities to enroll in the health insurance plan. • Within 30 days of your appointment date (deductions will be taken retroactively to cover you from the first day of employment). • During the annual open enrollment period in August of each year. Not automatic; must fill out form and present valid documentation to Health Services. • An enrollment form • A copy of your Social Security card • A copy of Birth Certificate or Passport • Marriage Certificate (if filing spouse as a dependent) • These documents are also required for each dependent you want to include under this health insurance. Fill out health insurance waiver at the start of the semester to avoid being billed for SHIP. • Must waive SHIP before September 15th of each academic year. • Opportunity to waive only for Spring/Summer Semester between January 1st and February 15th. • International students are able waive this fee as soon as they receive their Social Security Card. Health insurance will start on the first day of your employment. • BUT you will not be officially enrolled until the Health Services Office receives the documents stated beforehand. • If your documents come in after the first day of employment, once they are received, your health insurance will be retroactive back to the first day of employment.
  • 10. UMR Fellows are eligible for UMR. But because Fellows are not technically employees of the University, they must pay for the total premium themselves. During the summer months that you are not on active payroll status, you must fill out a continuation of health insurance form in spring in order to continue coverage over the summer. In order to be eligible for summer coverage, you have to be: 1. Coming back in the fall to the same type of assistantship, verified by their department. 2. Previously enrolled for health insurance coverage.Once you fill out the form and submit it, you will notice extra deductions taken out of your paycheck to pay for the summer months. You do not need to fill out a form if you will be continuously working and receiving a paycheck. Fellows who are not on summer appointment do not qualify for summer coverage.   • 11. Waiver Procedures (Other Insurance): Domestic students who have insurance from another source. Parent Spouse Other employer Not automatic; must fill out form and present valid documentation to Health Services. An enrollment form A copy of your Social Security card A copy of Birth Certificate or Passport Marriage Certificate (if filing spouse as a dependent) • These documents are also required for each dependent you want to include under this health insurance. Fill out a health insurance waiver at the start of the semester to not be billed for the regular student health insurance (SHIP). Must waive SHIP before September 15th of each academic year. Opportunity to waive only for spring/summer semester between January 1st and February 15th. When filling out the waiver, make sure your health insurance is active! • Otherwise, you will need to have your health insurance company provide a letter of coverage to the Health Services Office. If you waive SHIP and lose your alternative health insurance, the Health Services Office must be notified immediately. If you are a full-time matriculated student, they can put you on SHIP retroactively.
  • 12. Student Counseling and Psychological Services: • Located in the Student Health Center, also known as the Student Infirmary. • On the north end of campus near the Union Building and Kevin P. Lavalle stadium. • Check out hours here. • Operates as a triage unit. • When you walk in for the first time, you do not need an appointment. • First appointment: • Walk in during open hours. • Fill out the required paperwork. • Meet with a counselor that day. • The triage counselor will talk to you about what is going on and discuss with you what you are looking for. • From there, the triage counselor will see what services you are eligible for and set you up. • You can visit counselling for any reason, not only in a crisis situation. • Adjusting to campus, home-sickness, academic issues, social issues, etc.
  • 13. Student Counseling and Psychological Services: (cont.) After the first appointment, the Center has a variety of options for students seeking help: • Individual therapy • Group therapy • Consists of a socially related group. • New groups forming each semester • If there is a group of students that have the same interest or issue that they are dealing with, group therapies can be created around that issue. • Mindfulness meditation • Contact the counseling center during the first few weeks of the semester. • Takes a few weeks to get the meditation sessions going. If you are not comfortable using the campus counseling services, you can be referred to an off-campus clinician that is convenient for you. • The Counseling Center has a network of local and regional clinicians for student referrals. Counseling services are free and confidential • Not recorded on Academic Transcript • Parents and family are not notified. • There is a file that stays in the Counseling Center. • Release form must be signed in order to show this file to a professor or other staff member.
