3. Reporting
A report is a written presentation of factual information based on an
investigation.
The length of reports varies; there are short memorandum (memo) reports
and long reports.
In FTAT you will be asked to write a report as per the following format:

4. What is “Supervision”?
Supervision is formally defined as a relationship between the
supervisor and supervisee member(s) of a profession that!
Is evaluative,
Extends over time,
Serves to enhance the skills of the junior person,
Monitors the quality of the services offered by the junior person,
Acts as gatekeeping to the profession
5. Focal Test and Treat (FTAT) supervision
The district malaria officer or PHCU Director will meet the HEWs at the
health center or health post on a monthly basis.
The PHCU Director/supervisor will complete the FTAT monthly supervisor
checklist paper form to document and provide feedbacks immediately to
the HEWs.

6. Responsibilities
HEW and clinician/ laboratorian at the PHCU and Hospitals
Record information for all individuals consulted in health facilities standard
register books,
Document all malaria cases passively detected in the FTAT passive case
documentation (screening form),
source of positives could be from within the HP catchment area or outside,
using various communication tools
7. Responsibilities ….HEW and HWs at the PHCU and Hospitals
Determine if a case investigation can be done for each documented case as
per SCREENING FORM’s decision criteria
Consult respective PHCU surveillance focal person on the proposed course of
action
If case investigation is approved, it is initiated immediately without any delay
Conduct the case investigation within 3 days of the diagnosis date,

8. HEW is responsible for conducting the standard procedures
If the case happens out of the proper catchment area, notify respective PHCU
That case investigation will be assigned to respective HP /HEWs where the case
came from
Avail appropriate supplies for case investigation; provide treatment to your
patients & follow up services to all patients including those referred to the health
center by the HEWs during the FTAT activities

Patients positive for Pv, who received CQ & PQ during the FTAT &
who reside in the FTAT kebeles should get advices for any
hemolysis symptom to report and visit nearby health facilities.
Patients positive for malaria during the FTAT and who reside in
the FTAT kebeles will be followed for their adherence to
antimalaria drug by HEWs

PHCU Director / PHEM Focal
Supervise and support HEWs field work activities virtually or
physically
Contact HEWs to follow on documentation of case and initiated a
case investigation
Monitor the HEWs to ensure the implementation of case
investigation according to the SoP

12. Responsibilities…
Zone/ Woreda malaria officer
Supervise health center supervisors/surveillance focal person
Case report consolidation from higher (public/private) HFs
Maintain periodic contact with the supervisor or surveillance focal person at PHCU
Monitor the supervisor/surveillance focal person to make sure they are supervising
the HEWs and ensuring that case investigations are implemented in accordance with
SOP,
13. Responsibilities…
Zone/ Woreda malaria officer
Conduct regular meetings with all supervisors/surveillance focal persons under
their jurisdiction and complete the FTAT DHO/ZHD CHECKLIST FORM, and
Conduct regular meetings with all HEWs under their jurisdiction, ZHD reviews
completeness of all documentation.
Supervise HEWs, supervisors/ surveillance focal persons, and DHOs,
Maintain periodic contact with each DHO, and
Compile weekly progress reports to share with RHB and NMCEP
14. Responsibilities ……RHB, MoH/NMCEP, EPHI, EPSA and national partners
Update the Sop accordingly
Conduct quality assurance, molecular analysis …etc.
Ensure all case investigation implementation is done in accordance with the SoP
Organize field work, logistics, and personnel
Conduct regular meetings with DHO, ZHD …
Observe the case investigations in the field on a quarterly/bi-annually basis
Program managers and officers review incoming data to ensure fieldwork is going as per
the SoP; and give a timely feedback
