 Click “Home” at the top left hand corner of the
screen
 On the left hand side, hover mouse over “Locker”
 Click “My Forms”

o Firstand last name of the safety officer traveling with the club for the
event and their phone number
o FIRSTAND LASTNAME ALL OF THE MEMBERS ATTENDING THE
EVENT-Drivers and the safety officer(s) need to be included in this list
 Indicate“agree” to “Club Officer agrees to inform club members about sport club travel policies and procedures, this includes
club representation of good sportsmanship. Please inform Assistant Director ASAP of any travel problems of injuries.”
 Provideyour firstand last name and the date
AFTER A TEAM TRAVEL FORMHAS ALREADYBEEN SUBMITTED
