5. Before Childbirth
 Registering in early period of her pregnancy.
2 – 3 months (7% discount)
4 – 6 months (5% discount)
7 – 9 months (down payment 30% with 3% discount)
 Installment will be paid every month.
 Available for both Sectio and Partus Normal
*If there are any unexpected problems, patients will be
charged an additional fee.

6. After Childbirth
 Down payment 30%.
 Monthly installment.
 Available for both Sectio and Partus Normal.
 There is no interest for installing.

7. Requirements
To do the installment program, patients are required to
attach:
1. Photocopy of Identity Card
2. Bank Statement
3. Salary Slip (optional)
4. Guarantor (Special Case) by providing all required
documents as same as patients.
5. Collateral (optional)

8. Late Fee
 Late fee Rp. 150.000,-/month.
 Fee waving at the first time of late payment.
 Email and telephone for reminding patients to pay.
 Pay online or pay in hospital directly.
 If patients make the late payment more than 3 months,
Mayapada Hospital has a right to terminate the contract.

9. Settlement of Non Performing Loans
Sale of the collateral under hand, collateral as
defined by finance department (if any).
