a smooth transition.

6. TOP TEN TIPS FOR TRANSITIONING TO SECONDARY SCHOOL
Attend open days for secondary schools you are interested in- speak to the SENCO there.
If possible arrange a separate appointment with the SENCO at your preferable secondary school
to discuss your child’s SEN.
Find out what transition plan the secondary school offer. (Including Extended Transition)
Have a first and second choice of schools
Ensure your paperwork is in date order and that you have any relevant reports and that they are
all within the last 12 months.
Think about how your child will travel to school and will they require travel training.
7. Year 6 EHCP’s MUST be FINALISED naming the school
by
15th February 2018

Look around colleges and FE Providers.
•
Read prospectuses
•
Talk to the SEN Team
•
Plan, Prepare, Plan, Prepare

Find out what courses are on offer – involve your young person in the decision making.
•
Find out how many days a week the courses are for – not full time in most cases.
•
What will your young person do on days they are not in college/FE
•
Think about additional support they will need.
•
Plan Year 11 Annual review well in advance, be prepared – though this should have been discussed since
year 9 (PfA)
•
Think about how social care and health can support and what transition may be happening, if any within
those services.
•
Draft plans should be with you by January 2018 to allow time for consultation with the college/FE and
yourselves.
