• Using your smartphone - follow instructions to take the Needs Mapper.
• Your instructions should have a student name and date of birth. Enter the
student name and date of birth.
• Take the Needs Mapper (think of one of your 12th grade students who’s
still in high school that will be transitioning).

• In Groups, look at the data on your tablet and answer the following questions:
• What do you see in the data?
• What decisions would you try to make when you look at needs assessment, based on your role,
what type of actions would you take with this data?
• What services should we focus on?
• Brainstorm on services.
