Value Based Care:
Healthcare is moving from a fee‐for‐service to value‐based care reimbursement model. At the
heart of this are Quality Measures, which measure the effectiveness of health centers in
providing value‐based care. The measures are rolled across various programs like QPP/MIPS,
CPC+, PCMH, HEDIS etc.
Implementing a data platform is critical to proactively track scores and improve data quality
to maximize funding. Unlike others, Unity PHM does not just show computed measures,
rather we have a defined step‐by‐step workflow enabled process that measures, tracks and
helps optimize care management.
Measure Engine:
Our proprietary measure engine: Measures, Tracks and Optimizes scores across various
Quality Measures. The measure engine helps health centers get a handle on their data quality
by rolling up numbers into measures on a monthly basis. The embedded workflow helps
approach year‐end reporting in a systematic month‐to‐month process.
