This document provides a template for students to complete a book report or graded reader sheet. It includes fields to summarize the plot in 5-10 lines, describe main characters, when and where the story took place, whether they would recommend it to a friend, and rate it from 1 to 10. Guidelines at the bottom advise students to take their time reading and revising the report.

Some advice:
 Take your time to read the book. Write down some notes at the same time if necessary.
 Put your ideas in order before writing them. Write simple, short sentences and use
structures that you have studied.
 A good layout is always important.
 It can be handwritten but make sure it is easy to read.
 You should use white sheets of paper.
 Revise your mistakes when your teacher shows the report back to you.
