Applying Quality Improvement ToolsThis week, begin collecting data for your Personal
Improvement Project. Consider: What will you measure? When? How will you measure it?
How will each of the measures contribute to an understanding of your improvement? Use
the Data Collection Plan Template to ensure you have a good system in place for collecting
data (e.g., tracking on your phone, by hand in a notebook, etc.). You will collect data for your
Personal Improvement Project in Weeks 2 through 5. Journal: Personal Improvement
Project Outline Before engaging in any given project, it is helpful to have a plan. An outline
allows you to map out the steps of your project and foresee any challenges you may
experience along the way. It also can serve as a checkpoint. As you are collecting and
analyzing your data in later weeks, you may want to return to your outline and check that
you are addressing what you had intended. For this Journal, you will create an outline of
your Personal Improvement Project to guide you both in carrying out the project and in

reduce fall incidences by 60% by applying nursing fall prevention practices that are tied to
the fundamental goal of improving the patients wellbeing and functions. This is based on
the awareness that instead of achieving zero fall rates at the expense of other health care
priorities, the goal should be on keeping fall rates as low as possible. In essence, achieving a
fall incidences reduction by 60% will be the result of balancing many priorities (Vincent &

process you are studying?The process being studied is the value of a nurse managed
standardized assessment of fall risk factors in identifying and targeting patients at high risk
of falls for closer monitoring to reduce fall incidences (Vincent & Amalberti, 2016).Who are
the people involved?The persons involved are nurses who will administer the standardized
assessment of fall risk factors to patients upon being admitted into the facility.Where does
the process start and end?The process begins when the patient first gets into the facility and
only ends upon discharge from the facility.Part 3: MeasurementWhat are you
measuring?Changes in fall incidences will be of interest. Any changes will be assumed to be
the result of focus on the patients identified in the assessment (Perry, Potter & Ostendorf,
2016).How are you collecting the data?The data will be collected through fall incidence
reports at the facility.Part 4: ChangesWhat changes do you anticipate making based on what
the data reveal?It is anticipated that applying the standardized assessment of fall risk
factors will help in identifying high risk patients who will be targeted by the fall prevention

long-term?The project results will help in informing policy changes at the facility. The
project results will act as evidence for recommendations on how to reduce fall incidences at
the facility for the long term.How might the experience of this personal project inform a
quality project in nursing?The experience will help with understanding how to conduct a
quality project in nursing. It will help with understanding who to involve in the project,
from whom and how to get approvals, how to collect data, and how to evaluate the data. In
addition, the project will help with engaging other stakeholder to involve them in nursing
