6. Re-Cap: As soon as possible, but within 24 hours after you are injured at work, you need to: 1. Notify someone and complete a Team Member Incident Report. 2. Sign a HIPAA Medical Authorization form. 3. Receive a Physicians Report to take to the Approved Medical Provider.

It’s very important that you do so because making the report covers you for further medical treatment if necessary. Hilton has selected Approved Plan Medical Providers to provide you treatment in case of a work injury. They have been chosen for their ability to provide quality medical services.
If your injury requires a specialist , the Plan Medical Director will select the most appropriate medical specialist for you.
Be sure to follow the Approved Medical Provider’s treatment plan for your injury. Their job is to get you better, which is what we want too! If you have trouble with appointments or have any other difficulties with your medical treatment or injury, please call the Claims Adjuster right away for assistance.
The Approved Physician completes the report, and you need to return it to the appropriate person at your hotel or location. The Hilton team will work hard to accommodate any work restrictions that the Approved Medical Provider may give you during your recovery period.

If you have problems, or are not satisfied with your benefit payments, or how your injury claim is handled - you have rights! First, There is a formal appeals process if you believe you are not receiving the proper benefits. The Claims Adjuster can explain how this is done if it comes up for you. It is also explained in your Summary Plan Booklet. Second, the program includes a mandatory arbitration procedure to resolve any other injury-related disputes between you and Hilton quickly and fairly. A skilled arbitrator will hear both sides of the situation and come to a final and binding decision.
If you have any questions, please contact your HR manager or the person designated to coordinate your workplace injury claims. If you are a new employee you will get a Summary Plan Booklet explaining the program and will be asked to sign an Acknowledgment Form. Lastly – be safe!
