- The response to these programmes by health care professionals and by the general public has demonstrated the motivation to improve their knowledge and awareness of falls and bone health. A similar approach in raising awareness to and management of other geriatric syndromes to reduce the incidence and disabilities associated with them is possible. An eLearning education resource on incontinence and a programme on pressure ulcer prevention and management have been developed with potential for future development in other areas
- Forever Autumn Community of Practice was established in January 2013 as there was a lot of interest from healthcare professionals in the work that was going on in relation to bone health and falls prevention in St Mary’s. Interest was shown from across the spectrum of care and among all disciplines. Sharing of what works well across organisations and prevents re inventing the wheel. Thare are currently 74 members of the COP
- Community setting – engaged with HCP & facilitated education on bone health and falls to healthcare professional, community groups Acute setting – HCPs from differing acute settings collaborated through COP and share good practice and support implementation of new measures Residential setting - HCPs from differing residential care settings collaborated through COP and supported the implementation of Forever Autumn into their units Intellectual disabilities – Recognised there was no education resource to support bone health and falls awareness in ID group and collaborated with them to develop Happy Bones
- Happy Bones & Best Foot Forward were developed as part of the COP. Evidence shows 7/10 people with an intellectual disability have reduced bone density. There was no education specifically for this group. Through engagement with ID services and service users Happy Bones was developed and implemented in 2015/2016. It has been widely adopted across many services nationally and there is interest in it internationally
