How will Care Transition services work at Amada?
• Patients will be assessed to determine if they are
appropriate for transition coaching based on their illness,
their support system and the willingness to manage their
own health. (cognitive awareness)
• Care transition coaching supports patients and families
and provides a continuity of health care as patients
transfer between different locations or different levels of
care within the same location.

• The patient and family/other support systems are
empowered to advocate on their own about their health
concerns.
• Red flags/warning signs of their illness are discussed.
• Medication reconciliation is examined (discharge papers
are compared to current meds in the home- these are
often different and creates confusion for the patient)

• Assurance of the physician follow up appointment within
48 hours(post facility) (Research has shown that once
past the 48 hour window, readmission is likely to occur)
• Community resources are offered for non medical
needs/requests, ie. support groups, medication
assistance, referrals to area agencies on aging.

Motivational Interviewing – what is it?
Motivational interviewing is a form of collaborative
conversation for strengthening a person's own motivation
and commitment to change. It is a person-centered
communication style for addressing the common problem of
ambivalence about change by paying particular attention to
the language of change. (Using the patients language and
level of education)
