Chief complaint/ History of Presenting illness:
c/o intermittent fever since 1 week
c/o abdominal pain, headache since 5 days
c/o nausea and vomiting since 2 days
c/o redness of eyes & blood stained sputum since morning
Previous history:
Patient was treated with Tab. Augmentin 625 bd & Tab.
Sinarest tds for last 4 days at local hospital, Ramanagaram
Past MedicationHistory/Allergy:
Nothing significant

Diet- Mixed
Appetite- Decreased
Sleep- Decreased
Bowel β Regular
Bladder- Normal
Habits- None
Family history:
no f/h/o DM, HTN, ASTHAMA, TB
OBJECTIVE
Physical examination :
Vital signs:
BP - 122/80mmHg
RR β 20cpm
PR β 88bpm
Temp β 102 F.
10. Advice to patientο§ The main cause for the Dengue fever is Dengue virus
spreaded by Mosquito. So Insecticides should be used
to avoid mosquitos.
ο§ Eliminate stagnant water that serve as mosquito
breeding sites at home, workplaces and their vicinity.
ο§ Using mosquito nets at home and patients need to be
kept under mosquito netting until the second bout of
fever is over and they are no longer contagious.
ο§ Cover overhead tank to prevent access to mosquitoes.
ο§ Dispose of garbage on a regular basis.
ο§ Store trash in rodent-proof containers, and clean the
containers often.
ο§ Avoid unnecessary till fully recovers.
ο§ Diet: low fat, low fibre, non-irritated, non-carbonated.
Noodle soup may be given.
