enhance their self-management and promote independence. New topics are
presented each week and build on the previous session for maximum outcomes.
"Your Life, Your Health" is Louisiana's implementation of the Stanford Chronic
Disease Self-Management Program and sponsored by the Governor’s Office of
Elderly Affairs.
There is no charge for the workshop, but there must be at least 15 participants

Feedback from leaders of 2 workshops:
"Participants all felt the workshop had made a huge difference in how they will
manage their chronic conditions. One participant read us the letter she wrote to
her doctor. Several mentioned how impressed they were with the book. All talked
about the power of action plans and how they will continue to use them to break
goals into achievable tasks. They were pleased with the certificates and we had a
wonderful party afterwards. Can't wait to start the next one!"
Susan, Leader in Baton Rouge.
"All told us they really enjoyed the class and got a lot out of it. They want us to visit
them at the senior center to check on them. They said they learned a lot form
each other and made some new friends. Actually we learned from them also. It
was a great experience and looking forward to the next class."
