The “Your Life, Your Health" workshop for Jefferson Parish - East Bank starts
Monday, April 11th from 9:00 am - 11:30 am and meets at the same time every
Monday for 6 weeks at
                         Jefferson Senior Center (The Pink House)
                         4518 Jefferson Hwy.
                         Jefferson, LA 70121
"Your Life, Your Health" is an evidence based workshop developed by Stanford
University for people living with chronic conditions (or caregivers) on how to
enhance their self-management and promote independence. New topics are
presented each week and build on the previous session for maximum outcomes.
"Your Life, Your Health" is Louisiana's implementation of the Stanford Chronic
Disease Self-Management Program and sponsored by the Governor’s Office of
Elderly Affairs.

There is no charge for the workshop, but there must be at least 15 participants
enrolled to offer the workshop. Workshops are interactive and have been very well
received by the participants who have already attended. Please visit our brand

about the power of action plans and how they will continue to use them to break
