This document discusses the importance of reliability in organizations. It emphasizes the need for a proactive, top-down reliability culture with a positive attitude among reliability personnel. It also stresses the importance of in-house expertise in problem solving and root cause analysis, noting that software alone is not enough and that expertise and culture are also required to ensure reliability.

Global Energy shift : Oil to Solar - The greatest energy revolution of 21st c...MAHENDRA KUMAR RASTOGI
US withdrawal from the Paris Agreement : Is it not the matter of shame and irresponsible behavior of US to international community while whole Europe , China & India are supporting this movement of change from fossils to renewable solar energy.
"Wake up call " to all operators & Regulators of Ageing petroleum industries. MAHENDRA KUMAR RASTOGI
The document warns that aging assets in petroleum industries are dangerous and require immediate attention to maintain operations safely. It references failures at a Richmond refinery and regulators fining PG&E $1.6 billion as examples of issues caused by aging infrastructure. The document calls for learning from past failures to avoid future problems from outdated energy facilities.
Why Globally Future of Future of petroleum industries is in doldrum ?MAHENDRA KUMAR RASTOGI
The petroleum industry globally is struggling due to lower crude oil prices, competition from liquefied natural gas, shale oil and gas, and renewable energy like solar power. Stricter emissions and spill regulations, an aging workforce and equipment, and higher costs are also challenges. However, some Asian countries like India and Indonesia may continue growing their petroleum consumption since per capita use is still low and they import much of what they need.

A rupture in a Ф52" pipe upstream of a bend at the Tesoro Refinery on April 2, 2010 caused six fatalities. An investigation found the rupture was due to thinning of the pipe wall and over pressurization, which are symptoms of high temperature sulfidation and CLSCC (corrosion under low stress conditions). The document warns that a focus on quick fixes based only on symptoms without conducting a root cause analysis, fast repairs just to resume production, and rewarding staff for attending to problems in a reactive manner can develop a reactive staff culture and reactive role models. This type of culture is prone to catastrophic failures.
The document discusses issues with aging equipment in refineries and the importance of proactive management. It notes that aging equipment, if not properly maintained, can lead to accidents that kill workers. An example is given of a 2010 accident at a Tesoro refinery that killed six people. The document emphasizes the need for an integrated approach to plant integrity from start to finish. It also stresses that becoming a proactive culture requires significant and sustained effort from management to overcome organizational inertia toward reactive approaches. Failure to maintain management focus and commitment could allow a proactive culture to revert back to a reactive one.
This document discusses the importance of being aware of failure risks and consequences in complex systems and having appropriate barriers, monitoring, and hazard controls. It also outlines five strategic goals for achieving integrated plant integrity from initial operation through decommissioning. Finally, it emphasizes that transitioning from a reactive to a proactive culture requires significant ongoing management commitment to overcome organizational inertia.
The document discusses the importance of reliability in the oil and gas industry given aging assets, tighter regulations, and environmental protection threats. It notes that aging equipment must be properly maintained or it could fail catastrophically. Several examples of failures are provided, including a pipeline rupture that resulted in six fatalities. The document emphasizes taking a proactive rather than reactive approach to asset integrity from initial design through decommissioning. It states that significant management commitment is needed to sustain a proactive safety culture long-term.

Equipment at an oil refinery was aging and not being properly maintained, resulting in a catastrophic accident that killed six people. The document discusses the need to improve reliability for aging industrial plants to prevent dangerous failures through proper maintenance and repairs. Attention must be paid to safety as equipment gets older.
