Flu Consent packets went home last week. PLEASE READ
CAREFULLY!
The consent has two pages (front and back) that require
signatures and must be completed in ink. Incomplete
forms will result in the vaccine not being given. PLEASE
LOOK CAREFULLY AT THE SECTION ON THE CONSENT
THAT ASKS ABOUT RECENT VACCINES!!!! If your student
has had an MMR or Varicella vaccine twenty-eight (28)
days prior to December 4, 2014 or has an appointment for
one of the above vaccines twenty-eight days after the
above date, your child will not be eligible for the flu mist.
Please mark “flu shot” on the preference section. If you
have any questions, please call your school nurse.
