How to Apply?
Complete IRS Form 8850.
Pre-screening of personnel and requesting certification that a newly recruited employee is a member of the WOTC target
group are both handled using Form 8850 (Pre-Screening Notice and Certification Request).
Complete the ETA Form 9061.
The worker's unique qualities are listed on Form 9061 (Individual Characteristics Form). These comprise demographic
information and other specifics that reveal whether or not the applicant is a member of a WOTC target group. Without salary
records for unemployment insurance, the ETA Form 9175 (Long-Term Unemployment Recipient Self-Attestation) should be

Send all forms in
The state workforce agency must receive the paperwork within 28 days of the new hire's start date. The application has the

Receiving a decision
Each WOTC application will get a final determination from the state workforce agency. There could be a requirement for
further data or documentation. An employer can file a tax credit claim with the IRS after certification.

Typically, an employer will submit Form 5884 to claim the tax credit (Work Opportunity Credit). Instead, the company should
complete Form 5884-C (Work Opportunity Credit for Qualified Tax-Exempt Organizations Hiring Qualified Veterans) if it is
tax-exempt and hiring from the veteran target group.
