● Led the development and implementation of a medical record review process including selection of
providers, creation of letters and correspondence, medical record review, and tracking of all aspects
of the audit process
● Compiled and analyzed data from a variety of sources in order to prepare summary reports and
findings for presentation to quality committees. Designs and/or refines quality reports
● Assisted with the review and resolution of quality of care concerns including investigation, gathering
documentation, contacting providers, and updating tracking systems
● Supported development and completion of QI documents such as annual work plans, evaluations,

The Colorado Neurological Institute (CNI) is a local nonprofit organization that provides a multidisciplinary
care approach for patients who suffer from a neurological disorders including stroke, MS, TBI, Parkinson's,
and a variety of other chronic neurological disorders.  
Accomplishments
● Coordinated yearly international medical trip to help alleviate hearing loss and diseases of the ear
● Coordinated the Cochlear Implant Assistance Program that provides brand new devices to worthy
individuals across the country
● Coordinated annual nationally recognized summer camp for children with cochlear implants and
their families from across the United States and Canada
● Coordinated all other education events at CNI including neurological conferences, symposiums,
support groups, and professional development events including arranging volunteers
● Track record of successful grant submissions
● Met daily with stroke survivors at Swedish hospital to ensure proper discharge plans
● Provided home visits to at risk stroke survivors to ensure individuals are getting proper follow up
● Develop and facilitate support groups for patients and/or caregivers
Dual Diagnosis Therapist​, Community Reach Center
Thornton, CO — December, 2008-October 2012
Community Reach Center is a non-profit that is the primary provider of mental health services to low-income
families and individuals in Adams County, Colorado. 
Accomplishments
● Provided case management, counseling and other clinical interventions to residents and families as
appropriate to the position
● Prepared treatment plans, discharge plans and follow-up care programs, provided therapeutic crisis
intervention and emergency services as required
● Participated in the review of clinical issues and program policies/procedures and monitored
activities of daily living (personal care, food preparation, laundry, etc.) for residents and provided

For six summers, I worked as counselor and unit leader at a diverse camp which serves kids who suffer from
critical to degenerative illnesses. I was in charge of leading counselors and volunteers and was responsible
