Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

СЕРЕДИНЫ 60-Х ГОДОВ XIX — НАЧАЛА XX ВЕКА
Рассматривается деятельность земских учреждений Курской губернии в обла-
сти здравоохранения. Показано, что здравоохранение населения как необязательная
повинность земских учреждений накладывала отпечаток на их деятельность в этой
сфере, но с конца 80-х гг. XIX в. оно стало одним из основных направлений.
Ключевые слова: здравоохранение, Курская губерния, земская медицина, фель-
дшер, разъездная система, акушерская помощь, оспопрививание, строительство
больниц.
Актуальность темы определяется потребностью исторической науки в объ-
ективном изучении отечественной истории. В настоящее время в стране продол-
жает реализовываться национальный проект «Здравоохранение». Отрадно, что
цели и направления, которые в нем сформулированы, были выдвинуты и реали-
зовывались на практике еще земскими учреждениями. Это укрепление здоровья
населения России, снижение уровня заболеваемости, инвалидности, смертности,
а также развитие профилактической направленности здравоохранения и повыше-
ние доступности и качества медицинской помощи1. Таким образом, осмысление
опыта, накопленного земской медициной, важно для организации современно-
го здравоохранения, в руководстве которым должны учитываться элементы как
общегосударственного, так и местного подхода.
В контексте региональной истории большой интерес представляет изучение
деятельности земских учреждений Курской губернии в сфере здравоохранения.
Следует отметить, что деятельность земских учреждений Курской губернии,
связанная с заботой о здоровье населения, слабо разработана в отечественной
историографии и на региональном уровне.
История земской медицины Курской губернии долгие годы не являлась пред-
метом специального исследования, хотя ее отдельные аспекты и конкретные сю-
жеты в той или иной степени затрагивались в контексте изучения истории зем--

ских учреждений и истории медицины на всех этапах историографии .
Однако только в последнее время появились работы, в которых рассматри-
вается становление земской медицины как уникальной формы предоставления

медицинской помощи населению губернии .
При этом нам удалось привлечь новые документы из Государственного архи-
ва Курской области (ГАКО), раскрывающие именно это направление деятельно-
сти земских учреждений Курской губернии. Это указ императора Александра II
о передаче медицинской части ведомства государственных имуществ в ведение

2. земства, отчеты врачебного инспектора о деятельности земств в области здра-

воохранения, отчеты о состоянии медицинской части в губернии . Особую цен-
ность представляют опубликованные источники — журналы заседаний очеред-
ных и экстренных губернских и уездных земских собраний, материалы губерн-
ских съездов земских врачей и обзоры Курской губернии.
Курская губерния ко времени введения земских учреждений в 1865 г. пред-
ставляла собой типичный аграрный регион и имела административное деление
на 15 уездов. В 1862 г. в Курской губернии сельское население составляло 1 млн

638 тыс. Лечебная сеть губернии состояла из 14 больниц в уездных центрах и
одной губернской в Курске, находившихся в ведении приказа общественного
призрения. В 1865 г. во всех больницах насчитывалось 458 коек, т. е. одна койка
приходилась на 3 831 человек. Состояние этих учреждений не отвечало элемен-

тарным требованиям гигиены. Всем им требовался капитальный ремонт . Непо-
средственно в сельских населенных пунктах лечебных учреждений не было, сле-
довательно, их население оставалось без медицинской помощи.
Врачей было недостаточно — по 1 или 2 на каждый уезд (уездный и (или) го-
родовой). Они в основном жили в городах и изредка выезжали в деревню. В селах
было небольшое число фельдшеров, особенно ротных. Уровень их профессио-
нальной подготовки оставлял желать лучшего. Таким образом, функционировала
разъездная система предоставления медицинской помощи, которая носила слу-
чайный характер. Лишенное медицинского обслуживания население, особенное
сельское, прибегало к помощи знахарей и костоправов.
Систематически случались вспышки эпидемий. Например, в 1855 г. во время
эпидемии холеры, которая длилась 1,5 года, заболело 21 012, а умерло 9 202 че-
ловека, т. е. 43 % всех больных7. Фактически отсутствовала система санитарных
мероприятий. Население нуждалось в качественной медицинской помощи.
1 января 1864 г. «Положение о земских учреждениях» получило силу закона.
По «Положению» в качестве необязательной повинности земствам разрешалось
участие, «преимущественно в хозяйственном отношении, в попечении о народ-
ном здравии и благотворительных учреждениях»8. Земствам передавались боль-
ницы и богадельни от приказа общественного призрения. Финансовой состав-
ляющей служил т.н. приказной капитал, как правило, передаваемый не в виде
наличных денег, а в процентных бумагах. Земствам разрешалось устанавливать
дополнительные сборы с населения на медицинские нужды и принимать частные
пожертвования.
Кроме больниц по указу императора от 6 марта 1867 г. земству была передана
врачебная часть и фельдшерская школа от ведомства государственных имуществ.
Врачебная часть включала медицинский персонал, инструменты и лекарства9.
Управление земской медициной в губерниях возлагалось на земские управы.
Осуществление всех мероприятий по медицине и общественному призрению
законодательство предписывало вести в рамках прежних медицинских уста-
вов. Надзор за этой деятельностью осуществлял правительственный врачебно-
административный орган губернии—врачебное отделение Курского губернского
правления, которое возглавлял врачебный инспектор. Так, И. Ф. Тихомиров про-
водил ревизии земских лечебных учреждений и контролировал действия земств

3. по организации медицины в губернии. Иногда эту функцию врачебный инспек-
тор осуществлял совместно с губернатором.
Практическая деятельность земских управ в области устройства медицины в
губернии, и 'особенно в уездах, зависела от целого ряда факторов: наличия фи-
нансовых средств, социального состава земской управы, санитарного благополу-
чия местности и т. д. По данным Б. Б. Веселовского/всего на медицину в 1868 г.

земствами было ассигновано 1,2 млн р., или 8,3 % сметы . А в Курской губернии
на медицинскую часть уездными земствами было потрачено более 34 тыс. р., или
5,2 % сметы. Губернское земство тратило свои средства — на губернскую боль-
ницу 9 018 р. и фельдшерскую школу 4 044 р., что составляло менее трети всех

уездных земских затрат . Большинство уездных земств поначалу ассигновали
незначительные средства на медицину.
В 60-70-е гг. в работе земств у. гласных и врачей не было единых взглядов
на организацию медицинской помощи населению. Поэтому одной из самых на-
сущных проблем становления земской'медицины являлся поиск оптимальной
системы ее организации. В этот переходный период в Курском земстве сохраня-
лась разъездная система. Она подкреплялась тем, что земства не желали тратить
средства на наем врачей. Так, к 1869 г. на службе 15 уездных земств было всего 10
земских врачей, 7 уездных и 3 городовых. Из них 6 приходилось на 3 уезда — по
2 врача в каждом (Льговский, Корочанский и Обоянский уезды). По одному врачу
было в Рыльском, Белгородском и Новооскольском уездах. Разные причины за-
ставили покинуть земскую службу некоторых врачей. Так, Дмитриевское земское
собрание уволило земского врача в связи с тем, что его работа не приносила ни-
каких результатов.
Суджанский, Фатежский, Грайворонский, Старооскольский уезды вообще за
4 года даже не имели «особых» земских врачей. Не было врача и в Курском уез-
де. Его функции выполняли 2 фельдшера. Таким образом, на 15 уездов Курской
губернии приходилось всего 20 врачей и 66 фельдшеров. Подавляющая же часть
этих врачей проживала в городах. В их обязанности входило выезжать в уезд 1-2
раза в месяц12.
Построение медицинского дела представляло собой деление уезда на участки,
в которых имелись амбулаторные пункты, обслуживаемые фельдшерами. Кро-
ме них в селах открывались приемные покои на 4-5 кроватей. Так, например, в
Тимском уезде было 4 покоя. Они располагались в следующих селах: Сувалоч-
ном, Рогосцах, Конареве, Озерах. Эти населенные пункты находились на боль-
шом расстоянии от уездного центра (соответственно 35, 25, 40 и 15 верст). В них
в основном работали фельдшеры, в распоряжении которых находился набор ме-
дицинских инструментов и аптечки с лекарствами, которые они должны были
раздавать бесплатно населению. В связи с нехваткой врачей управы нанимали
фельдшеров, содержание которых было дешевле. Так, среднее жалованье врача
составляло 1 000 р., фельдшера — 180 р. Но и по мере появления врачей в даль-
нейшем приоритет все равно отдавался фельдшерам13.
Деятельность фельдшеров контролировали врачи, в обязанности которых вхо-
дил объезд фельдшерских пунктов. Однако зачастую контроль со стороны врачей
и управ был фиктивным. В связи с этим разъездная система породила в Курской

4. губернии, как и в других земских губерниях, «фельдшеризм». Даже в 1890 г. чис-
ло самостоятельных фельдшерских пунктов в Курской губернии достигло 90. В
губерниях центрального Черноземья их насчитывалось еще больше — в Тамбов-

ской 104, а в Воронежской 158 . Часто фельдшеры превышали свои полномочия.
От этого возникали несчастные случаи. Например, врачебный инспектор Тихо-
миров в своем отчете губернатору о состоянии медицины в губернии отмечал,
что фельдшеры злоупотребляют своим положением, вымогая деньги за бесплат-
ные лекарства, принадлежащие земству. И далее он подчеркивает, что «земства,
с одной стороны, производят необязательные затраты на предмет сельской меди-
цины, чем уже заслужили благодарность общества за ее бесплатность, но с дру-

гой — фельдшерская помощь причиняет вред населению» .
Сельское население, преимущественно крестьяне, к врачам относилось с осто-
рожностью и даже опаской, видя в них чиновников, подобно уездным исправни-
кам. Это обстоятельство влияло на степень обращаемости к врачам. Из общего
числа населения уезда обычно к врачам обращалась только незначительная часть
больных. Так, в Белгородском уезде в 1885 г. у врачей было 13748 посещений, а
у фельдшеров 28 111, т. е. в 2 раза больше. В дальнейшем, когда врачи пприобре-
ли авторитет в глазах крестьян, это соотношение стало меняться в противопо-
ложную сторону. Уже в 1889 г. у фельдшеров было 20 532 посещений, а у врачей
31 285, т. е. в 1,5 раза больше16.
Также проблемным направлением земской медицины являлась организация
акушерской помощи населению. В деревне роженицы пользовались в основном
услугами повивальных бабок. Если при нормальных родах им необходимо было
только принять младенца, то при тяжелых бабки предпринимали «методики»,
основанные «на невежестве». Дмитриевский уездный врач в 1873 г. отмечал, что
«бабки заставляли рожениц лесть в хомут, а родственников встряхивать ее за
ноги и за руки»17. От такой помощи часто случались выкидыши. Количество ро-
дов, принятых акушерками, было незначительным, поэтому они занимались в
основном оспопрививанием.
Оспопрививание населения шло нормально, но за счет участия самого населе-
ния и на средства крестьян. В связи с нехваткой кадров оспопрививателями были
обученные врачем или фельдшером крестьяне. Первые 15 лет населению приви-
валась т. н. гуманизированная лимфа (с руки на руку), но из-за частых случаев
переноса сифилиса от этого способа пришлось отказаться. Было принято решение
прививать детритную сыворотку, которую получали при помощи привития мо-
лодых телят. Привитием стал заниматься только специальный персонал — фель-
дшеры и акушерки. Таким образом, необходимо было решать эти проблемы.
С середины 1870-х гг. важная роль в решении этих вопросов стала принадле-
жать губернским съездам врачей, которые поднимали широкий спектр проблем:
переход от разъездной к стационарной системе, организация борьбы с эпидемия-
ми, борьба с «фельдшеризмом» и др.
В рассматриваемый период в Курской губернии было проведено 10 съездов.
I съезд был открыт в 1875 г. по ходатайству губернского земского собрания Мини-
стру внутренних дел. По итогам работы этого съезда было выработано заключе-
ние: «Признать систему объездов участков не только бесполезной, но и вредной,

5. ввиду потери врачом времени, которое он мог бы производительно употребить
на пользу народного здравия». По мнению самих врачей, этот съезд имел больше
теоретический характер в связи с тем, что на нем присутствовало мало членов

уездных управ .
В 1882 г. на съезде врачей вновь прозвучало положение об осуждении разъ-
ездной и переходу к стационарной системе предоставления медицинской помо-
щи как более рациональной для сельской местности. Также было рекомендова-
но создание коллегиальных органов для решения текущих вопросов врачебно-

санитарного дела уезда — врачебных или санитарных советов .
Первый совет в губернии функционировал еще с конца 70-х гг. в Фатежском
земстве. В начале 80-х гг. они были открыты при Корочанском, Щигровском, Кур-
ском и Новооскольском земствах. В дальнейшем они появились и в остальных зем-
ствах. Также при губернском земстве была создана санитарная комиссия в 1882 г.,
которая впоследствии была переименована в 1884 г. в губернский врачебный совет.
Губернский врачебный совет должен был изучать санитарное состояние и разра-
батывать предложения об улучшении земской медицины в рамках всей губернии.
В ведении совета находились все медицинские учреждения губернского земства.
Совет назначал и увольнял врачей, провизоров и высший хозяйственный персонал
закрытой баллотировкой20. Но позже прерогатива назначения и увольнения врачей
была изъята из ведения совета и передана губернской управе.
После организационного оформления земско-медицинских учреждений неод-
нократно поднимался вопрос о расширении сети врачебных участков. В Курской
губернии в 1890 г. было 60 врачебных участков, а на службе земства состояло 69
врачей, т. е. на 1 уезд приходилось в среднем 4 врача. Напротив же, в Воронежской
при 47 участках трудились 52 врача, а в Тамбовской при 55 служили 59 врачей, т. е.
соответственно в среднем также 4 врача21. С одной стороны, полученный результат
схож, но с другой, он условен, т. к. Курская губерния имела большее количество уез-
дов по сравнению с Воронежской и Тамбовской. В последних было по 12 уездов.
В этих участках было недостаточно сельских больниц. Назревал вопрос о не-
обходимости их строительства. Однако к 1896 г. Фатежское земство Курской гу-
бернии вообще не имело в сельской местности никаких стационарных лечебных
учреждений. В других уездах наряду с земскими существовали частные больнич-
ки при фабриках и имениях помещиков. В городах насчитывалось 31, а в уездах
29 лечебных учреждений, из них 27 содержалось на средства земства22.
В 1896 г. был утвержден план строительства земских сельских больниц в гу-
бернских межуездных и уездных медицинских участках. По плану строительство
шло за счет уездных и губернского земств пополам. С одной стороны, й большин-
стве этих больниц отсутствовали специальные отделения (родильные, заразные
и т. д.), но с другой — их появление позволило децентрализовать помощь труд-
нобольным, которых больше не нужно было отправлять в губернскую больницу.
Следует отметить, что больницы были построены за счет земских средств, вместе
с тем частичную помощь оказывали меценаты.
Период с 1890 по 1905 г. работы земских органов самоуправления явил в свет
разработку и реализацию других крупномасштабных проектов по организации
медицинской помощи населению.

6. Прежде всего, следует отметить создание оспопрививательного института в
1891 г. для выработки оспенного детрита для прививки жителям всей губернии.
С целью уменьшения радиуса участков и предоставления помощи населению, ко-
торое находилось на стыке уездов и в основном было лишено даже амбулаторной
помощи, с июля 1891 г. было открыто 6 губернских межуездных участков, они со-
держались на средства губернского земства. Участки охватывали 13 из 15 уездов

губернии .
Большое значение имела постройка в 1896-1905 гг. психиатрической колонии
в деревне Сапогово на 1000 коек для лечения и призрения душевнобольных в 10
верстах от г. Курска, что позволило разгрузить помещения губернской больницы.
Губернское земство воспользовалось правом на 50 % субсидию для улучшения

дела за призрением душевнобольных .
В 1898 г. губернское земство, не дожидаясь решения правительства об от-
крытии центральной фельдшерской школы в г. Харькове, решило возобновить
деятельность фельдшерской школы в Курске. Она была закрыта еще в 1884 г. по
«причине больших затрат и нехватки помещений для обучения и жилья учени-
ков». Кроме фельдшеров здесь начали готовить акушерок. Ученицы обучались
акушерству в лечебнице Общества Курских врачей, которой руководил выдаю-
щийся земский врач В. И. Долженков25.
Однако земские органы самоуправления Курской губернии так и не смогли
ввести санитарную организацию. По нашему мнению, для этого были все усло-
вия. В 1889 г. было открыто медико-статистическое бюро губернского земства
для систематизации материала по заболеванию населения и выработке рекомен-
даций по их профилактике. Сведения в бюро доставлялись от участковых зем-
ских врачей. В 90-х гг. XIX в. — первом десятилетии XX в. обсуждался вопрос в
уездных земских управах о возможности введения института санитарных врачей
и на чей счет они должны содержаться. Единого мнения так и не было принято.
Из 15 уездов 6 были против ее введения. К 1910 г. только в г. Курске и уезде были
санитарные врачи26.
Таким образом, в 1914 г. в сельской местности Курской губернии насчитыва-
лось 90 больниц, при их полном отсутствии в 1865 г. Лечебная сеть городов также
увеличилась с 15 больниц до 41 (в. г. Курске до 11). Коечная сеть всех лечебных
учреждений увеличилась в 6,7 раза и достигла внушительной цифры 3 183 крова-
тей. Пользовалось ими 43 717 человек, а умерло 1 757. Амбулаторных больных по
губернии зарегистрировано 1 853 674; в эту цифру вошли поступающие с фронта
больные и раненые воины.
Медицинский персонал также увеличился. Всего в это время в Курской гу-
бернии работало 147 врачей, в том числе неземских (15 уездных и 10 городовых);
в городах — 56 врачей, а в уездах — 91. Вспомогательный персонал находился в
основном при врачах. Число фельдшеров увеличилось до 298 по сравнению со 103
в 1870 г. Женского вспомогательного персонала было в 2 раза меньше, т. е. число
фельдшериц, акушерок и повивальных бабок составило 14727.
Все это свидетельствует о том, что уже в последней трети XIX в. медици-
на прочно вошла в число основных направлений земской деятельности Курской
губернии. Несмотря на скудность средств, административные препятствия, не-

7. хватку врачей и медперсонала, она способствовала организации национальной
системы обслуживания как городского, так и сельского населения.
