Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Van een hormoonzalf
tot lichttherapie, er
bestaan verschillende
methoden om aanmaak
van pigmentcellen te
stimuleren of afbraak ervan
tegen te gaan. Eén daarvan
is minigrafting. Dr. Ludmila
Nieuweboer is een van
de weinige dermatologen
in Nederland die deze
methode toepast – vaak
met succes. “Een goedkope
en eenvoudige ingreep”,
zegt ze.
Weer kleur
in de witte huid
dankzij minigrafting
Minigrafting houdt in dat tiental-
len minuscule gepigmenteerde
huidstukjes worden verplaatst
naar de te behandelen
vitiligoplek. Daar groeien ze naar elkaar toe,
waarna de witte huid vaak weer een zo goed
als normale huidkleur krijgt.
Mensen denken soms dat het hier gaat om
een heuse transplantatie. ‘’Is ziekenhuisop-
name nodig? Of een donorpersoon die een
stukje huid afstaat?’’ Dr. Nieuweboer kan ze
snel geruststellen: ‘’Het gaat om een goed-
kope en eenvoudige ingreep, die wereldwijd
al jarenlang wordt uitgevoerd. Er komt geen
donorhuid aan te pas. De behandeling wordt
poliklinisch gedaan, verspreid over één of
meerdere sessies.’’
Appelboortje
Bij minigrafting wordt gebruikgemaakt van
een zogeheten biopteur, een soort mini-ap-
pelboortje. Daarmee verwijdert de arts onder
lokale verdoving zeer kleine rondjes (biopten)
van 1 tot 2 millimeter uit een gepigmenteerd
huidgebied. ‘’Meestal kies ik voor het huidop-
pervlakte van de heup, billen of bovenbeen,
omdat de zogeheten donorplek daar wordt
bedekt door kleding en later niet in het oog
springt”, zegt Nieuweboer.
Vervolgens worden de bruine biopten één voor
één naar de witte acceptorplek verplaatst.
Ook die locatie is verdoofd. Op een onderlinge
afstand van zo’n vijf millimeter worden daar
kleine gaatjes geboord waar de biopten pre-
cies inpassen. Een behandeling duurt ongeveer
30 tot 45 minuten, waarin circa veertig tot
zestig stukjes huid worden getransplanteerd.
‘’Iedere plek is natuurlijk anders gevormd, per
keer gaat het om een oppervlakte tot circa
honderd vierkante centimeter.’’
Zowel de donor- als de acceptorplaats wor-
den na de transplantatie afgedekt met een
doorzichtig kleefverband. Nieuweboer: ‘’Op de
donorplek kan de pleister er al na drie dagen
af, op de acceptorplaats blijft deze een week
zitten. In die periode kunnen de biopten goed
hechten.’’
Na verwijdering van het verband ziet de
donorplek er meestal nog rood uit, sowieso
is de kleur van de nieuwe huid dan nog niet
dezelfde als van de huid eromheen. Om de
pigmentatie in de geplaatste biopten te stimu-
leren is aanvullende lichttherapie nodig. Een
gezichtsbruiner kan hiervoor worden gebruikt,
maar ook lichtbakken of lichtcabines, thuis of
in het ziekenhuis. Het duurt weken voor de
behandelde plek langzaam maar zeker een
egale, normale huidkleur krijgt.
Mitsen en maren
Zeker niet iedereen komt in aanmerking voor
minigrafting. Voorwaarde is dat de te behan-
delen plek al zeker een jaar stabiel is, zich
niet meer uitbreidt. Is dat wel het geval? ‘’Dan
moet eerst de vitiligo gestabiliseerd worden,

WIE IS LUDMILA NIEUWEBOER?
Van 1995 tot 2012 werkte Ludmila Nieuweboer bij de SNIP,
de Stichting Nederlands Instituut voor Pigmentstoornissen,
bij het AMC in Amsterdam. Destijds was ze ook enkele jaren
gedetacheerd bij het Nederlands Kankerinstituut – Antoni
van Leeuwenhoek. Sinds 2012 werkt ze bij het Zuwe Hof-
poort Ziekenhuis in Woerden.
Nieuweboer heeft zelf vitiligo en paste minigrafting toe op
haar eigen huid. “In eerste instantie leek dit aan te slaan,
maar helaas bleek mijn vitiligo toch niet stabiel. Om die
reden ben ik ermee gestopt.’’
dat kan met hulp van hormooncrème en/of
belichting.’’
Dr. Nieuweboer vervolgt: ‘’Lange tijd zagen
mensen op tegen belichting uit angst huid-
kanker te ontwikkelen. Inmiddels weten we
dat die vrees ongegrond is, als mensen met
vitiligo een zogeheten UVB 311nm-therapie
volgen. Deze behandeling zorgt, in tegenstel-
ling tot vroegere lichttherapieën (zoals PUVA)
juist voor een snellere stabilisatie en spontane
repigmentatie. Dit lichtspectrum biedt minder
verbranding en een aanzienlijk kortere belich-
tingstijd.’’
Geeft de dermatoloog groen licht voor mini-
grafting? Dan vindt eerst een proeftransplan-
tatie plaats. De specialist neemt twee of drie
biopten uit de gepigmenteerde huid en plaatst
die in de acceptorplaats. Slaat dit aan, dan kan
een afspraak voor de werkelijke transplantatie
worden gemaakt. Naar verwachting groeien
de biopten na verdere behandeling en belich-
ting uit tot een egaal geheel.
Kinderhoofdjes
Keerzijde is dat niet bij alle mensen de
proefbehandeling aanslaat. De overgeplaatste

WIE IS LUDMILA NIEUWEBOER?
Van 1995 tot 2012 werkte Ludmila Nieuweboer bij de SNIP,
de Stichting Nederlands Instituut voor Pigmentstoornissen,
bij het AMC in Amsterdam. Destijds was ze ook enkele jaren
gedetacheerd bij het Nederlands Kankerinstituut – Antoni
van Leeuwenhoek. Sinds 2012 werkt ze bij het Zuwe Hof-
poort Ziekenhuis in Woerden.
Nieuweboer heeft zelf vitiligo en paste minigrafting toe op
haar eigen huid. “In eerste instantie leek dit aan te slaan,
maar helaas bleek mijn vitiligo toch niet stabiel. Om die
reden ben ik ermee gestopt.’’

verbranding en een aanzienlijk kortere belich-
tingstijd.’’
Geeft de dermatoloog groen licht voor mini-
grafting? Dan vindt eerst een proeftransplan-
tatie plaats. De specialist neemt twee of drie
biopten uit de gepigmenteerde huid en plaatst
die in de acceptorplaats. Slaat dit aan, dan kan
een afspraak voor de werkelijke transplantatie
worden gemaakt. Naar verwachting groeien
de biopten na verdere behandeling en belich-
ting uit tot een egaal geheel.
Kinderhoofdjes
Keerzijde is dat niet bij alle mensen de
proefbehandeling aanslaat. De overgeplaatste

4. Jarenlang liet Maggie van Spall (65) haar
vitiligo – over het gehele lichaam verspreid
– voor wat het was. “Ik groeide op met de
boodschap dat je tegen die witte vlekken
helemaal niets kon doen. Ik maalde er ook
niet om, ik ben wie ik ben.’’ Tot haar oud-
ste dochter veertien jaar geleden zwanger
raakte. “Juist in die periode vertelde een
dermatoloog me dat ik met lichttherapie
wél iets kon ondernemen. Ook al stapte ik
altijd probleemloos de sportzaal en zelfs de
sauna binnen, toch hapte ik toe. Waarom?
Omdat de vitiligo bij mij begon na mijn
eerste zwangerschap, ruim veertig jaar
geleden. Stel dat mijn dochter dit ook zou
overkomen! Ik wilde haar meegeven dat er
wel degelijk iets tegen te doen is.”
Maggie begon met lichttherapie als voor-
bereiding op een eerste behandeling met
minigrafting, een jaar later. Anno 2016
heeft haar bovenlichaam een complete me-
tamorfose ondergaan: vanaf het schouder-
gebied (voor en achter), de hals en nek tot
en met haar gezicht. Was het gezicht van
de Indonesisch-Chinese Maggie in 2004 al

nagenoeg pigmentloos, inmiddels is dat
weer bijna net zo lichtbruin als in haar
jongere jaren.
Snel herstel
Hoe heeft ze de minigrafing-behandeling
ervaren? ‘’Het herstel na de transplanta-
tie verloopt snel”, vertelt Maggie. “In de
eerste dagen wordt de huid nog bedekt
met een groot doorzichtig kleefverband.
Dan is het vooral heel belangrijk dat je
niet transpireert. Sporten en het verrich-
ten van grote inspanning kun je daarom
maar beter even laten. In die periode van
herstel lijkt het nog of je een schaafwond
hebt, al ontstaat er geen korst. Vervol-
gens volg je een korte periode licht-
therapie. ‘Dan zie je de bruine plekjes
langzaam groeien.’
Tevreden
Maggie kijkt tevreden terug op de behan-
delingen. Hoewel haar dochter na de be-
valling niet te maken kreeg met vitiligo,
koos ze er toch voor om de minigrafting
voort te zetten. “Het beschermde me, ik
verbrandde niet meer zo in de zon. Maar
ik krijg nog steeds nieuwe witte plekken.
Dan ga ik weer naar dr. Nieuweboer om
te overleggen wat op dat moment de
beste aanpak is.”
biopten groeien dan niet uit of worden wit,
wat erop kan duiden dat de vitiligo toch
niet stabiel is. Ook kan het zijn dat de kleur
van de nieuwe ‘pigmenteilandjes’ niet goed
matcht met de omliggende huid. Een derde
reden kan zijn dat de huid na de transplan-
tatie licht hobbelig wordt. Dit noemen we
het ‘kinderhoofdjeseffect’. “Oorzaak daarvan
is de aanwezigheid van enig littekenweef-
sel rond en onder de biopten, dat de stukjes
huid lokaal iets omhoog kan duwen.
Hiertegen is niet altijd iets te doen”, legt
dr. Nieuweboer uit. Bekend is dat dit effect
vooral kan optreden na een behandeling in
de hals of in het gezicht. “Het is daarom erg
belangrijk dat de dermatoloog biopten in het
donorgebied heel vlak afneemt. Dan praat
je over niet meer dan ongeveer een kwart
millimeter dikte.”
Dit zijn allemaal redenen om je als patiënt
heel goed te informeren voordat je aan de
minigrafting begint. “En dat is lastig”, weet
dr. Nieuweboer. “Omdat nergens overzich-
telijk wordt vermeld in welke ziekenhuizen
minigrafting-specialisten te vinden zijn die
ervaring hebben met transplantatie. Enkele
dermatologen die hiervoor eerder zijn opge-
leid bij de Stichting Nederlands Instituut voor
Pigmentstoornissen (SNIP, bij het Academisch
Medisch Centrum in Amsterdam), werken
inmiddels in verschillende ziekenhuizen. Om
kwaliteit en aanbod te verbeteren, worden
inmiddels wel extra artsen opgeleid.’’
Maggie:
