Performance Task: Scrapbook on Becoming a Responsible Adolescent
Objective:
The learners shall be able to make a list of ways to become responsible
adolescents prepared for adult life and manage the demands of teen
years through a creative and reflective scrapbook.
Instructions:
1. Create a scrapbook that highlights at least 10 practical and
meaningful ways you can become a responsible adolescent and
prepare for adult life.
2. For each way:
- Include a short explanation or description.
- Add a personal reflection or insight (Why is this important to you?
How do you plan to apply it?).
- Use images, drawings, magazine cutouts, or printed visuals to
creatively represent each point.
3. Organize your scrapbook with a cover page, table of contents, and
sections or themes (e.g., Responsibility at Home, in School, with
Friends, for Self).
4. Label all parts clearly and ensure proper grammar, spelling, and
neatness throughout.
5. You will present your scrapbook in class on the assigned date.

 1 short or long folder/bond paper (you may use colored or recycled
paper)
 Art materials (markers, crayons, glue, scissors, etc.)
 Old magazines, newspapers, or printed pictures
 Printed or handwritten reflections and explanations
 Any other materials that showcase your creativity and effort
 Note: Digital scrapbooks may be allowed upon teacher approval.
