Individual Application
Instructions
Dear Scouters and Parents;
Please insure that all the information is filled out completely and clearly on the individual application
form so as to avoid any delays in our travel plans.
On the application form we are asking for Canadian Passport Number, Canadian
Birth Certificate Number or Landed Immigrant Number and Status. This is
information that we must provide to the U.S. Customs and Border Protection before we
arrive as a pre-screening process. It makes crossing much easier and faster.
Valid Passport – The passport of a Canadian citizen must be valid through the intended duration of
their stay in the U.S. (As of January 2016). An expired passport may prompt a secondary inspection and entry
to the U.S. will be at the discretion of the border official. Citizens of other countries may be required to hold
passports that are good for up to 6 months after their intended departure date.
Children under age 16 may enter the U.S. or Canada using one of the following:
 U.S. or Canadian Passport
 Original or copy of Birth Certificate
 Naturalization Certificate
 Canadian Citizenship Card
 Consular Report of Birth Abroad
Children Traveling without Both Parents
It is strongly recommended that if only one parent is crossing the border with a child under age 18
that they have a Consent Letter from the other parent granting permission to take the child out of the
country. It is even more important if the child is traveling with a friend or relative without
either parent present.
Non-Canadian or US passport holders:
It is suggested that non-Canadian or US Passport holders obtain an ESTA ( http://esta.evisa-
travel.org/ ) even though we are travelling by land.
"All passengers traveling under the Visa Waiver Program are required to have an approved travel authorization prior to
traveling to the United States by air or sea. Even non-ticketed infants are required to have an approved travel
authorization, if they do not have a visa for travel to the United States. An application may be submitted by a third party
on behalf of a Visa Waiver Program traveler."
INSURANCE: It is your responsibility to obtain and have proof of out of
country medical insurance.
