Aetna is one of the largest Health Insurance providers in United States. Aetna mainly focused
on providing health insurance plans to corporate companies. Aim of EIE project is to provide
health insurance plans to individual members by creating enterprise service bus to integrate
various applications.
Aetna is using insurance product HRP (Health Rules Payer) from Health Edge for enrollment. It
bought BSwift tool for designing and creating web apps to enroll. Other applications which are
included in this integration are EDI, Active Health (Vendor), EDH (Enterprise Data Hub) built on
Mark Logic, in-house built CMA.
EIE layer comprises of integration tools like IIB, WTX, ODM, WSRR, WDP, BPM, WXS and Mark
Logic. Part of building EIE engine is enrolling members to insurance plans. Aetna uses industry
standard EDI X12/HIPAA message format and XML for communication between applications.
BSwift (web app) sends the X12 message which is intercepted by EDI gateway before landing in
IIB. IIB uses WTX maps for conversion between X12 and XML messages. Enrollment
information is then fed to HRP, EDH and Active Health applications. Other protocols which are
used include REST/HTTP and File.

CVS Caremark Corporation is an American retailer and health care company. CVS Caremark
provides pharmacy services through 7500+ retail stores, pharmacy benefit management, mail
order and specialty pharmacy division. It is headquartered in Woonsocket, RI. It is ranked 40th
Fortune Global 500 list and 13th
in US only Fortune 500.
The Specialty Connect is a strategic project for CVS Caremark to integrate different LOB’s of
CVS Caremark – RxConnect, Specialty and PBM. The Middleware layer is called Enterprise
Service Layer (ESL) which constitutes Datapower, IIB, WMQ and WSRR as main components.
ESL has 2 datacenters, East (Woonsocket, RI) and West (Scottsdale, AZ). Datapower is used as
security gateway and as a proxy service for Consumers of ESL and service providers to ESL.
Datapower is also used as service governance policy enforcement point. Connection between
ESL and end applications is SOAP over HTTPS. Security is enforced in the form of 2 way SSL
auth. Exception to this is QL/LINKS Mainframe applications where the connection is over WMQ.
Each datacenter has 2 Datapower appliances which are Self-Load Balanced using AO feature.
Datapower communicates with IIB over WMQ.
WMQ/IIB is configured as highly available clustered environment. WMQ hosts multi instance
queue managers and IIB hosts multi instance Integrations nodes. All transformation and
routing happen in IIB. Datapower and applications outside ESL always communicate with
Gateway qmgr.
WSRR is used for service Governance where business services are created and mediation
policies are attached. These policies are enforced in Datapower for service providers and service
consumers.
This is Webservices and Cobol/WMQ based integration. The project deals with retrieval of
patient information, updates of patient information and transfer of patient between LOB’s. Role
of ESL is to connect consumers RxConnect, Specialty and PBM with service providers EPH,
RxConnect, Specialty and PBM. EPH is central repository of patient information. End applications
RxConnect, Specialty and PBM act as both consumers and service providers. IIB plays major
role in transformation of messages between consumers and providers and aggregation of
responses from multiple providers using Aggregation nodes. It also involves AllergyCode cross
reference mapping where data is stored persistently in DB2 database and ExtremeScale built in
caching is used for better performance. Message exchange between ESL and QL/LINKS
application over MQ is in Cobol fixed length format which is handled using DFDL parser. IIB is
also used to enforce WSRR mediation outage policy for LINKS as service provider which
communicates over MQ.
Extensive performance test has been conducted using HP LoadRunner. The system is designed
to support 200 tps. This includes 5% growth every year for next 5 years. JMeter is used to do
internal performance testing within ESL.

It is ranked 40th Fortune Global 500 list and 13th in US only Fortune 500. The Specialty Connect is a strategic project for CVS Caremark to integrate different LOB’s of CVS Caremark – RxConnect, Specialty and PBM. The Middleware layer is called Enterprise Service Layer (ESL) which constitutes Datapower, IIB, WMQ and WSRR as main components. ESL has 2 datacenters, East (Woonsocket, RI) and West (Scottsdale, AZ). Datapower is used as security gateway and as a proxy service for Consumers of ESL and service providers to ESL. Datapower is also used as service governance policy enforcement point. Connection between ESL and end applications is SOAP over HTTPS. Security is enforced in the form of 2 way SSL auth. WMQ hosts multi instance queue managers and IIB hosts multi instance Integrations nodes. All transformation and routing happen in IIB. Datapower and applications outside ESL always communicate with Gateway qmgr. WSRR is used for service Governance where business services are created and mediation policies are attached. These policies are enforced in Datapower for service providers and service consumers. This is Webservices and Cobol/WMQ based integration. The project deals with retrieval of patient information, updates of patient information and transfer of patient between LOB’s.
