directions for your convenience.
For the appointment, I will need you to bring some required documents on your side to make
sure our meeting goes as planned. The following documents are needed:
 Last three years of tax returns
 Current bank statements (savings, checking, business, etc.)
 Bills for all debts owed (including student loan’s)
 Current statements for investment accounts
 Proof of all assets owed (titles of vehicles, mortgage documents, jewelry appraisals,
etc.)
 Life insurance policies with cash value
 Checkbook
 Competed “customer information form,” which is also attached.
It is important that all documents are brought to the meeting. If you have any questions, please
