For the appointment, I will need you to bring some required documents on your side to make
sure our meeting goes as planned. The following documents are needed:
Last three years of tax returns
Current bank statements (savings, checking, business, etc.)
Bills for all debts owed (including student loan’s)
Current statements for investment accounts
Proof of all assets owed (titles of vehicles, mortgage documents, jewelry appraisals,
etc.)
Life insurance policies with cash value
Checkbook
Competed “customer information form,” which is also attached.
It is important that all documents are brought to the meeting. If you have any questions, please
